吸入沙美特罗氟替卡松对COPD稳定期患者肺功能及气道炎症的影响

摘    要:目的探讨吸入沙美特罗氟替卡松对慢性阻塞性肺疾病(COPD)稳定期患者肺功能及气道炎症的影响。方法将诊断明确的46例中、重度COPD稳定期患者随机分为观察组(n=23)和对照组(n=23),对照组给予健康教育、合理氧疗、解痉平喘、化痰等治疗措施。观察组在以上基础上给予沙美特罗氟替卡松50/500μg,每次1吸,每天2次。分别观察治疗1年后患者的FEV1、FEV1/FVC(%)、FEV1占预计值百分比等肺功能指标及静脉血中细胞因子IL-8、TNF-α水平。结果观察组患者FEV1、FEV1/FVC、FEV1占预计值百分比等通气功能指标较治疗前均有不同程度提高(P均〈0.05);患者外周血中细胞因αIL-8、TNF-α水平较治疗前有显著降低(P均〈0.05)。结论长期吸入沙美特罗氟替卡松可明显改善COPD患者肺功能指标降低气道的慢性炎症反应,与对照组相比有显著差异。

Abstract:Objective To observe effects of salmeterol/fluticasone propionate inhalation on pulmonary function and airway inflammation in chronic obstructive pulmonary disease (COPD) patients. Methods Total 46 cases of definite diagnosis of moderate to severe COPD patients during stable period were divided into two groups randomly, the control group (23 cases) was given health education and treatment with rational oxygen therapy, antispasmodic, antiasthmatic, sputum and other therapeutic measures. The observation group was given salmeterol/flutieasone 50/500μg inhalation meanwhile, 1 suction each time, twice per day, besides the treatment and measures in the above. After 1 year,the data of pulmonary function, such as FEV1, FEV1/FVC (%), FEV1/FEV1 (expected)( % ), and the level of venous serum eytokine IL- 8, TNF-α were observed. Results FEV1,FEV1/FVC, FEV1 in observation group all increased than that before treatment ( P 〈 0. 05 ). Conclusion Salmeterol/ fluticasone propionate inhalant can significantly improve the pulmonary function and decrease airway inflammation in COPD patients during stable period compared with the control group.
