常规白内障超声乳化术和冷超声乳化术治疗老年性白内障的临床比较

目的:探讨常规白内障超声乳化术(连续模式、脉冲模式)与冷超声乳化术(爆破模式)术后早期的疗效比较,为选择一种更为安全有效的术式治疗老年性白内障提供理论依据。方法:分别对我院2007-11/2008-09的Ⅲ级以上核白内障患者做冷超声乳化术和常规白内障超声乳化术,并比较它们的临床效果。结果:两种手术均成功完成,冷超声乳化术较常规白内障超声乳化术在角膜内皮计数和平均超声时间这两项指标更优越,在中央角膜厚度及最佳矫正视力方面也较常规白内障超声乳化术相当甚至更好。常规白内障超声乳化术中脉冲模式各项指标较连续模式更优越,但某些指标的优越程度无统计学意义。结论:冷超声乳化摘除Ⅲ级以上核白内障加人工晶状体植入术,术后视力提高明显,无角膜损伤,较常规白内障超声乳化术同样甚至更为安全有效。

Comparison contrast common phacoemulsification with cold phacoemulsification

AIM:To discuss the comparison of the early postoperative efficacy of conventional phacoemulsification (linear mode, pulse mode) with the cold phacoe-mulsification (burst mode) .To select a more safe and effective surgical treatment of senile cataract and provide a theoretical basis.METHODS: From November 2007 to September 2008 nucleolus cataract (Ⅲ and above Ⅲ degree) underwent cold phacoemulsification and conventional phacoemul-sification in our hospital were retrospectively analyzed and the clinical effect was compared.RESULTS: Two types of surgery were successfully completed. The corneal endothelial count and average ultrasound time in cold phacoemulsification were superior to that in conventional phacoemulsification, the central corneal thickness and the best corrected visual acuity were also better in cold phacoemulsification. Conventional phacoemulsification pulse mode indicators were better than the linear mode, but some indicators did not have the superior degree of statistical significance.CONCLUSION: Visual acuity of cold phacoemulsification above Ⅲ degree cataract extraction with intraocular lens implantation improved significantly, and there was no injury to the cornea in cold phacoemulsification was the same with or even more safe and effective than conventional phacoemulsification.
