| 摘 要： | 目的探讨鞍区肿瘤的手术入路。方法回顾性分析应用额外侧经侧裂入路显微外科切除鞍区肿瘤48例的临床资料。结果大型及巨大型垂体腺瘤30例,全切19例,大部分切除8例,部分切除3例;鞍结节脑膜瘤10例,全切除7例,部分切除3例;颅咽管瘤8例,经终板入路全切除4例,部分切除4例。结论额外侧经侧裂入路应用显微外科技术,解剖外侧裂及鞍区诸脑池,能充分显露前颅底及鞍区结构,适用于大部分鞍区肿瘤手术。

| Abstract: | Objective To explore the operative therapy of sellae area tumors.Methods The clinical data of 48 patients with sellae area tumors treated microsurgically from external frontal lobe through lateral salcas in our hospital was summarized.Results Among the patients pathological diagnosis in thirty cases were pituitary adeaomas,the tumor was tottally removed in nineteen cases,subtotally in eighteen cases,partially in three cases.The tubercalam sellae miningiomas was in ten cases,among those,seven cases was totally removed,partially in three cases.The craniopharyngioma was in eight cases,the tumor totally removed and partially removed was two halves.Conclusion The microsurgically operative approach from external frontal lobe through lateral salcas is adaptable to most typies of tumors in sellae area,anatomizing the lateral salcas cisterna and those in sellae area,exposing clearly the structure of anterior skull base and sellae area. |
