心肌致密化不全病例与类似致密化不全病例的鉴别

目的探讨心肌致密化不全（Noncompaction of ventricular myocardium,NVM）与类似致密化不全病例的鉴别。方法将28例多次入院的心力衰竭病例初次住院时超声心动图与末次超声心动图进行比较。观察左心室壁内膜光滑程度、肌小梁、隐窝、收缩末期心肌最厚处非致密化心肌与致密化心肌比值（NC/C）等情况。结果 8例成人NVM发病初期及末次超声心动图均显示左心室内可见粗大肌小梁以及隐窝,末次NC/C平均值（2.07±0.12）;10例高血压心脏病（Hypertensive heart disease,HHD）及10例扩张型心肌病（Dilated cardiomyopathy,DCM）初期超声心动图未见肌小梁及隐窝,末次超声心动图提示左室可见增粗肌小梁,NC/C平均值〈2（P〈0.05）。结论超声心动图可以对心肌致密化不全及类似致密化不全进行鉴别,减少误诊率。

Comparison of noncompaction of ventricular myocardium cases with similar cases

Objective To investigate the diagnostic methods of echocardiographic morphology in patients with noncompaction of ventricular myocardium and noncompaction with similar cases.Methods Observed the smoothness degree of endocardium,myocardial trabeculation,recess,the ratio of most end-systolic myocardial thickness of noncompaction of myocardium and compaction myocardium(NC/C),and compared the initial 28 cases of hospital echocardiography and echocardiography at the end of times.Results Both first and last echocardiography of 8 patients with NVM showed left ventricular muscle trabeculae can be seen,as well as large recess,NC/C average was(2.07±0.12);the early echocardiography of 10 cases of HHD(hypertensive heart disease) and 10 cases of DCM(dilated cardiomyopathy) did not show trabecular muscle and recess,and the trabecular muscle scan be seen in the last echocardiography,NC/Caverage<2(P<0.05).Conclusion Echocardiography can identify noncompaction of the ventricular myocardium and similar noncompaction,and reduce the misdiagnosis rate.
