炎症及凝血指标与急性脑梗死预后关系的临床分析

摘    要:目的探讨炎症和凝血指标对急性脑梗死预后和病情判断的临床价值。方法检测160例急性脑梗死患者超敏C反应蛋白(hs-CRP)炎症指标及凝血酶原时间、活化部分凝血活酶时间、凝血酶时间、纤维蛋白原、D二聚体凝血指标。根据1个月随访结果将患者分为恢复良好组和恢复不佳组,同时根据随访过程中的病情变化将患者分成进展性脑梗死组和非进展性脑梗死组。结果恢复不佳组hs-CRP水平(4.9±1.6)mg/L]显著高于恢复良好组(1.9±0.9)mg/L)](P〈0.01),恢复不佳组D二聚体水平(402±67)μg/L]显著高于恢复良好组(195±51)μg/L](P〈0.01);进展性脑梗死组hs-CRP水平(3.9±1.5)mg/L]显著高于非进展性脑梗死组(2.5±0.7)mg/L](P=0.03),进展性脑梗死组D二聚体水平(415±78)μg/L]显著高于非进展性脑梗死组(177±56)μg/L](P〈0.01)。结论早期检测hs-CRP和D二聚体等炎症和凝血指标有助于判断急性脑梗死预后和疾病进展。

Abstract:Objective To study the clinical value of serum inflammatory and hemostatic biomarkers in the prognosis of acute cerebral infarction.Methods High-sensitivity C-reactive protein(hs-CRP) inflammatory biomarker and hemostatic biomarkers including of prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT),fibrinogen and D-dimer within 24 h of onset in 160 consecutive acute ischaemic stroke patients were detected.These patients were divided into good recovery group and the poor recovery group according to 1 month follow up results.They were also divided into progressive cerebral infarction group and the non-progressive cerebral infarction group according to the evolution of the disease.Results Hs-CRP level of(4.9±1.6) mg/L in the poor recovery group was significantly higher than that of(1.9±0.9) mg/L in the good recovery group(P<0.01).D-dimer level of(402±67) μg/L in the poor recovery group was significantly higher than that of(195±51) μg/L in the good recovery group(P<0.01).Hs-CRP level of(3.9±1.5) mg/L the progressive cerebral infarction group was significantly higher than that of(2.5±0.7) mg/L in the non-progressive cerebral infarction group(P<0.05).D-dimer level of(415±78) μg/L in the progressive cerebral infarction group was significantly higher than that(177±56) μg/L in the non-progressive cerebral infarction group(P<0.01).Conclusion Early detection of hs-CRP,D-dimer and other inflammatory and hemostatic biomarkers is of high value in the prognosis of acute cerebral infarction.
