Abstract:
[Objective] To explore the clinical characteristic and occurrence rule of silent myocardial ischemia(SMI) and the clinical significance of SMI on 24 hour dynamic electrocardiogram(DCG).[Methods] Twenty-four hour DCG monitoring was performed in 82 cases of coronary heart disease(CHD) and was discussed according to the results with clinical features.[Results] There were 69 cases(84.15%) with altering of ST segment with ischemia,25 cases(36.23%) with symptoms and 44 cases(63.77%) with non-symptoms.Seven hundred and ninety-two times of myocardial ischemia were found,in which 641 were found with SMI,and 151 were found with symptoms.ST segment was down from 0.1 mV to 0.3 mV.The heart rate was faster in happening of myocardial ischemia than that before it.The frequency of SMI was highest between 6 am and 12 pm,and it was lowest between 0 am and 6 am.It consisted 22 minutes per time in SMI and 8 minutes per time in myocardial ischemia with symptom(P<0.05).[Conclusion] SMI is the most common manifestation in patients with CHD,and 24 hour DCG has the significant importance for monitoring SMI.
