102例初诊2型糖尿病患者胰岛素使用前后中医病理因素变化

目的观察厦门地区102例初诊2型糖尿病患者胰岛素治疗前后中医证素变化特点。方法收集符合纳入标准的102例初诊2型糖尿病患者的临床资料,采用证素辨证方法,观察胰岛素治疗前、治疗后6周及12周时证素的分布及变化特点。结果 102例初诊2型糖尿病患者治疗前病位证素以肾、肝证素居多。病性证素中,以热、阴虚、气虚、湿证素为主。治疗6周、12周后,仍以肾、肝证素频数及积分较高;实性证素居前两位的仍为热、湿证素,虚性证素前两位为阴虚、阳虚。结论随着治疗时间的延长,胰岛素可使初诊2型糖尿病患者证素分布发生变化,肾、热、肝、阴虚、湿频数下降,其中气虚频数及积分明显下降,而阳虚频数及证素积分升高。

Pathological Factor Changes before and after Insulin Application in 102 Pa-tients with Type 2 Diabetes at the First Visit in Traditional Chinese Medicine

Objective To observe the characters of syndrome factor changes in TCM before and af-ter treatment with insulin in 1 02 patients of type 2 diabetes at the first visit in Xiamen region.Methods The clinical data of 1 02 patients of type 2 diabetes at the first visit,in accordance with inclusive standard were collected.The syndrome factors and pattern/syndrome differentiation were adopted to observe the characters of syndrome factor distribution and changes before,in 6 weeks and 1 2 weeks after treatment with insulin sepa-rately.Results Among 1 02 patients of type 2 diabetes,kidney and liver syndromes were predominated;re-garding the nature of disease,heat,yin deficiency,qi deficiency and damp syndromes were predominated be-fore treatment.In 6 weeks and 1 2 weeks after treatment,the frequency and score of kidney and liver syn-dromes were quite high,heat and damp syndromes were at the top two in the category of excess syndrome,and yin deficiency and yang deficiency were at the top two in the category of deficiency.Conclusion With the treatment lasting,insulin changes the distribution of syndrome factor of the patients with type 2 diabetes.The frequencies of kidney,heat,liver,yin deficiency and damp syndromes are reduced,of which,the frequency and score of qi deficiency syndrome are reduced apparently and those of yang deficiency are increased.
