超声衰减成像定量诊断非酒精性脂肪肝的应用价值初探讨

摘    要:目的探讨超声衰减成像(ATI)技术定量诊断非酒精性脂肪肝(NAFL)的临床应用价值。方法回顾性分析110例NAFL患者,设为研究组,其中轻度35例,中度42例,重度33例,另选取35例肝脏正常患者作为对照组。所有患者均接受常规二维超声(US)检查,同时接受ATI检查,并需要在一周内接受过甘油三酯(TG)检验。比较分析研究组与对照组的衰减系数(AC),体质量指数(BMI)以及TG的差异性。进一步分析AC与US诊断结果,BMI以及TG的相关性。结果研究组与对照组间AC、TG均存在统计学差异(P<0.05),而BMI无统计学差异。AC与US结果、BMI、TG存在正相关性(P<0.05)。结论 ATI技术可以无创、定量诊断NAFL,对评估NAFL病变程度具有很好的参考价值,但仍需要进一步扩大研究来明确其临床使用价值。

Abstract:Objective To explore the clinical value of ultrasonic attenuation imaging(ATI) in the quantitative diagnosis of nonalcoholic fatty liver(NAFL). Methods A retrospective analysis of 110 patients with NAFL was carried out. The study group consisted of 35 mild, 42 moderate and 33 severe patients. All patients were examined by conventional two-dimensional ultrasound(US), ATI and TG(within one week). The differences of attenuation coefficient(AC), body mass index(BMI) and TG between the study group and the control group were analyzed. The correlation of AC and US diagnostic results, BMI and TG were further analyzed. Results There were significant differences in AC and TG between the study group and the control group(P<0.05), but no significant difference in BMI. AC was positively correlated with US, BMI and TG(P<0.05). Conclusions ATI technology can be used for noninvasive and quantitative diagnosis of NAFL, which has a good reference value for evaluating the pathological changes of NAFL, but further research is needed to clarify its clinical value.
