自发性食管破裂的早期诊断及治疗

目的：探讨自发性食管破裂的早期诊断及治疗。方法：回顾性分析我院2004年4月至2007年4月收治的自发性食管破裂患者9例,分析其临床表现及外科治疗方法。结果：早期开胸手术的6例患者治疗效果较好,其中痊愈5例,术后并发吻合口瘘1例。晚期手术患者2例,1例因术后并发脓胸及急性肾功能衰竭而死亡,1例自动出院。保守治疗1例因不能控制的严重感染和中毒性休克而死亡。结论：提高对自发性食管破裂的认识,早期手术是治疗成功的关键。

The Early diagnosis and reatments about spontaneous rupture of esophagus

Objective:To investigate the Early diagnosis and reatments about spontaneous rupture of esophagus.Methods:Retrospective analysis of 9 cases about spontaneous rupture of esophagus and theirs clinical features and surgical treatments from Apr 2004 to Apr 2007.Results: 5 patients of early surgery were cured in 9 patients,but one patient had post-operatice anastomotic leakage,one late surgical patient died of complication of empyema and acute renal failure,one late surgical patient abandoned the treatment.One patient had been treated by conservative therapy died because of uncontrollable severe infection and toxic shock.Conclusions: To increase awareness about spontaneous rupture of esophagus and early surgery is the key to successful treatments.
