超声弹性成像对BI-RADS 4类乳腺肿块的诊断

摘    要:目的 分析超声弹性成像技术在诊断乳腺影像报告与数据系统(breast imaging reporting and data system,BI-RADS4类乳腺肿块中的应用效果。方法 回顾性分析2020年6月至2022年5月浙江萧山医院收治的136例乳腺肿块患者的临床资料。比较不同性质乳腺肿块超声弹性成像参数,以病理结果为金标准,分析超声弹性成像诊断BI-RADS 4类乳腺肿块的价值,绘制受试者操作特征曲线(receiver operating characteristic curve,ROC曲线)并计算曲线下面积。结果 136例乳腺肿块患者的病理结果显示良性35例,恶性101例。乳腺恶性肿块的弹性指数、面积比、弹性应变比及弹性超声评分均显著高于乳腺良性肿块(P<0.05)。超声弹性成像诊断乳腺良、恶性肿块的敏感度、特异性及准确度分别为88.12%、82.86%、86.77%,与病理结果具有一致性(Kappa=0.629,P<0.05)。ROC曲线结果显示,超声弹性成像诊断BI-RADS 4类乳腺肿块的曲线下面积为0.810(95%CI:0.717~0.903)。结论 超声...

Diagnosis of BI-RADS type 4 breast masses by ultrasonic elastography
Abstract:Objective To analyze the application effect of ultrasonic elastography in the diagnosis of breast masses of breast imaging reporting and data system (BI-RADS) type 4. Methods The clinical data of 136 patients with breast mass admitted to Zhejiang Xiaoshan Hospital from June 2020 to May 2022 were retrospectively analyzed. Ultrasonic elastography parameters of breast masses with different characteristics were compared, and pathological results were used as the gold standard to analyze the diagnostic value of ultrasonic elastography for the breast masses of BI-RADS type 4. The receiver operating characteristic (ROC) curve was used to calculate the area of the curve. Results The pathological results of 136 patients with breast mass showed benign 35 cases and malignant 101 cases. The elasticity index, area ratio, elastic strain ratio and elasticity ultrasound score of malignant breast masses were significantly higher than those of benign breast masses (P<0.05). The sensitivity, specificity and accuracy of ultrasonic elastography in the differential diagnosis of benign and malignant breast masses were 88.12%, 82.86%, and 86.77%, respectively, which were consistent with the pathological results (Kappa=0.629, P<0.05). The ROC curve results showed that area under the curve for the diagnosis of BI-RADS type 4 breast masses by ultrasonic elastography was 0.810 (95%CI: 0.717-0.903). Conclusion Ultrasonic elastography can effectively diagnose BI-RADS type 4 breast masses. By observing the tissue hardness and elastography atlas of internal tissues and surrounding areas of the lesions, it is helpful for early differential diagnosis of benign and malignant masses.
