鸦胆子油乳注射剂联合化疗治疗胃癌的系统评价

目的:评价鸦胆子油乳注射剂联合化疗治疗胃癌的疗效和安全性。方法:计算机检索Pub Med,Embase,Medline,Cochrane图书馆,CNKI,万方,维普,CBM 8个数据库,采用Cochrane系统评价方法对纳入的随机对照试验(RCTs)进行方法学质量评价,并用Revman 5.2软件进行分析。结果:共纳入9项RCTs,共计594例患者。Meta分析结果显示,鸦胆子油乳注射液联合化疗组较单用化疗组能更明显地提高部分缓解率(PR)OR=1.74,95%CI(1.23,2.47),P=0.002],总缓解率(CR+PR)OR=2.45,95%CI(1.71,3.51),P0.000 01];并能显著改善患者生活质量(KPS评分10分)OR=2.73,95%CI(1.25,5.96),P=0.01]。化疗不良反应方面,联用组发生手足综合征、恶心呕吐、腹泻、白细胞减少、血小板减少、中性粒细胞减少的风险较单用组更低(P0.05)。结论:现有的研究表明,鸦胆子油乳注射剂联合化疗治疗胃癌较单用化疗治疗能更有效地提高患者部分缓解率、总缓解率及生活质量,化疗不良反应事件发生率则更低。受纳入研究质量限制和可能存在的发表偏倚影响,上述结论尚需更多高质量的随机对照试验加以验证。

Systematic Review of Yadanzi Oil Grease Injection Combined with Chemotherapy for Gastric Cancer

Objective: To evaluate the efficacy and safety of Yadanzi oil grease injection combined with chemotherapy in patients with gastric cancer. Method: Databases such as Pubmed, Embase, Medline, Cochrane Library, CNKI, Wanfang, VIP and CBM were searched for qualified randomized controlled trials (RCTs). Cochrane system evaluation method was used for the methodological quality assessment of included randomized controlled trials (RCTs),and Revman 5.2 software was used for analysis. Result: 9 RCTs involving 594 patients were included. The results of Meta analysis showed that in comparison with chemotherapy alone, the Yadanzi oil grease injection combined with chemotherapy could more significantly increase partial remission rateOR=1.74, 95%CI(1.23,2.47), P=0.002],overall remission rateOR=2.45, 95%CI (1.71-3.51), P<0.000 01]and could significantly improve the quality of life for patients (KPS score>10)OR=2.73, 95%CI (1.25,5.96), P=0.01]. In respect of chemotherapy adverse events, the risks of hand-foot syndrome, nausea and vomiting, diarrhea, leukopenia, thrombocytopenia, and neutropenia in the combinational therapy group were lower than those in chemotherapy alone group(P<0.05). Conclusion: The existing studies showed that compared with chemotherapy alone, Yadanzi oil grease injection combined with chemotherapy can more effectively improve partial remission rate, overall remission rate and quality of life, and the incidence of chemotherapy adverse events was lower in the combinational therapy group. For the quality restriction and possible publication bias of the included studies, more high quality and large sample RCTs are needed to verify the validity.
