经尿道膀胱肿瘤等离子切除术与电切术在膀胱肿瘤治疗中的应用

摘    要:目的:对比经尿道膀胱肿瘤等离子切除术(PKRBt)与电切术(TURBt)在膀胱肿瘤治疗中的临床价值。方法将2000年1月~2013年12月于我科行经尿道内腔镜手术治疗的282例膀胱肿瘤患者分为PKRBt组(132例)和TURBt组(150例),对比两组患者的临床资料、手术时间、术中失血量及闭孔神经反射发生率。结果两组手术时间、术中失血量比较无统计学差异(P>0.05);治疗侧后壁膀胱肿瘤时,PKRBt组闭孔神经反射发生率显著低于TURBt组(P<0.05)。结论 PKRBt与TURBt治疗浅表性膀胱肿瘤均安全有效,PKRBt的闭孔神经反射发生率较低,手术安全性较高。

Abstract:Objective To compare the clinical value of plasmakinetic resection of bladder tumor (PKRBt) and transurethral resection of bladder tumor (TURBt) in treating bladder tumors.Methods 282 patients with bladder tumors who had underwent transurethral endoscopic surgery from January 2000 to December 2013 were divided into PKRBt group (132 cases) and TURBt group (150 cases). The clinical data, operation duration, intraoperative blood loss and the occurrence rate of obturator nerve relfex of PKRBt group were compared with those of TURBt group.Results There were no significant differences between the two groups in terms of operation duration and intraoperative blood loss (P〉0.5). There were significant differences between the two groups in terms of the occurrence rate of obturator nerve reflex (P〈0.05) occurred during the operation process of bladder tumors which were located at the lateral posterior wall of bladders.Conclusion It is safe and effective to use PKRBt and TURBt to treat superifcial bladder tumors. The occurrence rate of obturator nerve relfex of PKRBt is lower than that of TURBt, which indicates that the operation safety of PKRBt is higher than that of TURBt.
