糖尿病视网膜病变激光光凝术后睫状体脱离的超声生物显微镜观察

摘    要:
目的观察糖尿病视网膜病变(DR)患者行视网膜光凝术后睫状体的形态学改变。方法 26例(29只眼)经检眼镜和荧光素眼底血管造影(FFA)检查确诊为增生前期、增生期DR需行全视网膜光凝术的患者。所有患眼首次光凝下方1/4象限视网膜,激光参数:激光点数400~450点,光斑直径250~300μm,标准Ⅲ级光斑。并于首次光凝术前和术后第2天行超声生物显微镜(UBM)检查,观察和分析光凝术后睫状体的形态改变。结果视网膜光凝术后第2天21只眼(72.4%)出现睫状体脱离。21只眼中17只眼(80.9%)为全周睫状体脱离,4只眼(19.1%)为部分象限脱离。结论 DR视网膜光凝术后睫状体脱离是暂时的、无严重后果的较为常见的眼部并发症。

Abstract:
Objective To investigate the tomographic changes of ciliary body after retinal photocoagulation for diabetic retinopathy.Methods Twenty-six(29 eyes)patients suffered from preproliferative and early proliferative diabetic retinopathy diagnosed by fundus examination and FFA were treated with panretinal photocoagulation.In the first session,the inferior quadrant of the fundus was treated with laser photocoagulation.Laser parameters: 400~450 burns of 250~300 μm spot size on the retina of standard grade Ⅲ intensity.The eyes were examined by UBM before,two days after the first session of photocoagulation to observe and analyze the changes in the ciliary body.Results Two days after retinal photocoagulation,ciliary detachment was found to be present in 21 eyes(72.4%).The whole circumference of the ciliary body was detached in 17of the 21 eyes(80.9%).The detachment was confined to several quadrants of the pars plana in four eyes(19.1%).Conclusion Ciliary detachment is a very frequent feature after sector photocoagulation in diabetic eyes.It is temporary and usually do not result in untoward complications.
