黄连素对2型糖尿病患者血清视黄醇结合蛋白4水平的影响

摘    要:目的探讨2型糖尿病(T2DM)患者黄连素治疗前后血清视黄醇结合蛋白4水平(RBP4)的变化。方法20例新诊断的T2DM患者经过饮食和运动治疗后血糖和糖化血红蛋白仍然不能达到良好控制者,予黄连素片0.9g/a口服治疗共16周,分别测定治疗前、后患者身高、体质量、腰围、臀围,测定空腹血糖(FBG)、TC、TG、游离脂肪酸(FFA)、空腹胰岛素(Fins)、糖化血红蛋白(HbAlc)、RBP4水平。根据公式分别计算BMI、腰臀比(WHR)、胰岛素抵抗指数(Homa—IR)。结果T2DM患者予黄连素治疗16周后BMI、WHR略低于治疗前,但差异无统计学意义(P〉0.05),FPG、TC、TG、FFA、HbAlC、Fins、RBP4、Homa—IR水平均明显低于治疗前(6.4±1.7)mmol/L比(7.5±2.0)mmol/L,(5.2±1.4)mmol/L比(6.0±1.5)mmol/L,(1.6±0.4)mmol/L比(2.0±0.6)mmol/L,(1.1±0.3)mmol/L比(1.4±0.4)mmol/L,(6.5±1.0)%比(7.2±1.1)%,(2.7±0.9)mU/L比(3.8±1.2)mU/L,(11.4±1.7)μg/L比(15.3±2.9)μg/L,0.9±0.3比1.1±0.4],差异均有统计学意义(P〈0.05或P〈0.01)。结论黄连素能明显降低T2DM患者的血糖、血脂水平,可能通过减少脂源性细胞因子RBP4的分泌而减轻胰岛素抵抗。

Abstract:Objective To study the effect of berberine on the concentration of retinol binding protein 4 (RBP4) in patients with type 2 diabetes mellitus (T2DM). Methods Twenty patients with newly T2DM whose fasting plasma glucose(FPG) and glycated hemoglobin (HbAlc) were undercontrol after diet and exercise conduction were enrolled in this study and were prescribed berberine(0.9 g/d) for 16 weeks treatment. The height, weight, waist and hip were measured before and after treatment. The serum was collected to examine the concentrations of FPG, total cholesterol ( TC ), triglycerides ( TG), free fat acid ( FFA ), fasting insulin ( Fins ), glycated hemoglobin (HbAlc), RBPd, the body-mass index(BMI), the waist-hip-ratio (WHR) and HOMA-insulin resistance index (HOMA-IR) were calculated according to the famula. Results The BMI and WHR in this study after 16th treat- ment were lower than those before treatment, but there was no statistical difference (P 〉 0.05). FPG, TC, TG, FFA, HbA1 c, Fins, RBP4 and HOMR-IR were lower than those before treatment (6.4 ± 1.7) mmol/L vs (7.5±2. 0) mmol/L, (5.2 ± 1.4) mmol/L vs(6.0 ± 1.5) mmol/L, ( 1.6 ±0.4) mmol/L vs(2.0 ± 0.6) mmol/L, (1.1 ±0.3)mmol/Lvs(1.4±0.4)mmol/L, (6.5 ±1.0)% vs(7.2±1.1)%, (2.7±0.9)mU/Lvs (3.8± 1.2) mU/L, ( 1 1. 4± 1.7) μg/L vs( 15.3 ± 2.9) μg/L, 0.9 ±O. 3 vs 1.1 ± 0.4 ] , there was statistical difference ( P 〈 0. 05 or P 〈 0.01 ). Conclusion Berberine can decrease the serum plasma glucose and lipid in patients with T2DM; it may improve IR by decreasing the RBP4 secretion.
