弧形切割吻合器在低位直肠癌保肛术中的应用初探

目的:评价弧形切割吻合器(凯途)在低位直肠癌保肛手术中的临床价值.方法:回顾分析2005年10月至2006年6月期间,18例患者在低位直肠癌保肛手术中应用弧形切割吻合器的临床资料.结果:18例患者均使用双吻合器行一期吻合.术后10 d出现肠瘘1例(5.6%),术后8 d出现直肠阴道瘘1例,1个月瘘口均未愈合而行横结肠造瘘.肛门坠胀不适6例,里急后重13例;日排便次数为5～10次8例,10次以上1例.1例出现严重尿储溜需放尿管.3个月后排便和排尿功能恢复接近正常.结论:弧形切割吻合器进行保肛手术安全可靠,操作简便,对患者术后排便、排尿功能影响较小.

Application of curved-cutter-stapler (Contour) in anus-preserving operation for low rectal cancer

Objective: To evaluate the clinical value of curved-cutter-stapler in anus-preserving operation for low rectal cancer. Methods: The clinical data of 18 patients with low rectal cancer, received anus-preserving operation using curved-cutter-stapler from 2005 to 2006 were reviewed retrospectively. Results: One patient had anastomotic leakage 10 days after operation and one patient had rectovaginal fistula 8 days after operation. Bowel dysfunction was found in the half of these patients, and there was one bladder dysfunction, but bowel and bladder function recovered about three months later. Conclusion: Curved-cutter-stapler is safe, reliable and easy to use in anus-preserving operation for low rectal cancer, with less bowel and bladder disfunction.
