不同离心条件下对不同人群凝血常规检测结果的影响

摘    要:
目的探讨不同离心速度和时间对不同人群凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)和纤维蛋白原(FIB)检测结果的影响。方法收集新生儿和婴儿新鲜血液标本共60例,另收集儿童及成人标本各60例,每例标本平均分成三份,分别以2 500 r/min离心15 min,3 500 r/min离心5 min和3 000 r/min离心10 min(对照组)。利用ACL-TOP全自动血凝分析仪及配套试剂测定血浆的PT、APTT、TT和FIB,同时应用Coulter LH 750全自动血细胞分析仪测定血小板数量。结果不同离心条件对成人凝血结果没有影响(P>0.05);2 500 r/min离心15 min和3 500 r/min离心5 min与对照组3 000 r/min离心10 min相比较,新生儿和婴儿PT、APTT、TT和FIB均缩短(P<0.05);儿童只有PT和APTT缩短(P<0.05),TT和FIB与对照组的差异无统计学意义(P>0.05)。结论乏血小板血浆的制备对于凝血常规检测结果的准确性及可靠性有重要影响,应建立适宜不同人群的乏血小板血浆制备的最佳离心条件。

Abstract:
Objective To investigate effects of different centrifugation time and speed on prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT) and fibrinogen (FIB) in different people. Methods 60 cases of newborns and infants, 60 cases of children and 60 cases of adults fresh blood specimens were divided into three groups of each case, which were respectively centrifuged with 2 500 r/min speed, 15 min, 3 500 r/min speed, 5 min and 3 000 r/min speed, 10 min (control). The PT, APTT, TT and FIB results were tested by ACL-TOP full-automatic blood coagulation analyzer. The platelet of the blood plasma were measured by Coulter LH 750 full-automatic blood cell analyzer. Results There was no effect on the result of the adults blood clotting under different centrifugal conditions. PT, APTT, TT and FIB in the group of 2 500 r/min speed, 15 min and 3 500 r/min speed, 5 min were lower than those of control group in newborns and infants(P〈0.05).PT, APTT were lower than those of control group in children(P〈0.05), but there was no significant difference between TT and FIB. Conclusion Best centrifugal condition of spent platelet plasma should be established for different people.
