摘 要: | 目的 探讨用红细胞平均体积(MCV)与红细胞体积分布宽度(RDW)值鉴别各型贫血的临床价值.方法 健康对照组100例,试验组患者167例,试验组又分为缺铁性贫血、轻型地中海贫血、叶酸缺乏及再生障碍性贫血组,静脉取血0.5~1.0 ml用EDTA-K2抗凝,应用BC3000型血细胞仪在2 h内测定.结果 健康对照组MCV(85±10.1)fl、RDW(12.5±1.8)%;缺铁性贫血组MCV(67.1±11.5)fl、RDW(22.5±5.4)%;轻型地中海贫血组MCV(65.3±13.5)fl、RDW(12.5±1.3)%;叶酸缺乏组MCV(95±21.2)fl、RDW(26.5±2.5)%;再生障碍性贫血组MCV(95±19.6)fl、RDW(12.5±0.9)%.MCV鉴别各型贫血均有统计学意义,RDW除轻型地中海贫血和再生障碍性贫血外均有统计学意义.MCV、RDW诊断缺铁性贫血敏感性为95.3%、特异性为96.8%;轻型地中海贫血的敏感性为93.2%、特异性为94.1%;叶酸缺乏敏感性为91.6%、特异性为95%.结论 MCV、RDW在诊断各型贫血的敏感性和特异性较好,可作为鉴别各型贫血的指标.

Abstract: | Objective To discuss the clinical value of MCV and RDW in identifying the types of anemia. Methods There were 100 cases in Healthy control greup,and 167 cases in experimental group. Experimental group was divided into iron-deficiency anemia,thalassemia,folate deficiency and aplastic anemia groups. Venous blood was collected 0.5 - 1.0 ml ,EDTA -K2 was antico- agulant,and BC3000 blood cell analyzer measured in 2 h. Results MCVofheahhycontrolgreupwas (85 ± 10.1) fl,RDWwas ( 12.5 ± 1.8 ) % ; MCV of iron deficiency anemia was (67.1 ± 11.5) t, RDW was (22.5 ± 5.4) % : MCV of light thalassemia was (65.3 ± 13.5) f1,f0)W was (12.5 ± 1.3)% ;MCV of folate deficiency was (95 ± 21.2) f1,RDW was(26.5 ± 2.5)% ;MCV of aplastie anemia (95 ± 19.6) fI,RDW was (12.5 ± 0.9)%. MCV to identify various types of anenda have statistical significance, and RDW have statistical significance outside the light - thalassemia and aplastic anemia. Sensitivity was 95.3% and specificity was 96.8% that MCV,RDW in diagnosis of iron deficiency anemia. Light sensitivity of thalassemia was 93.2% and specificity was 94. 1% ;sensitivity of folate lack was 91.6% and specificity was 95%. Conclusion Sensitivity and specificity of MCV and RDW in the diagnosis of various types of anemia are better. MCV and RDW can be, used as identification of various types of anemia indicators. |
