合并基础疾病的严重急性呼吸综合征患者临床特征分析

目的　探讨合并基础疾病的严重急性呼吸综合征(SARS)患者临床特征和实验室指标变化特点。方法　回顾性分析2003-03~06北京地区1 291例SARS患者的病历资料,对基础疾病的分布和有、无基础疾病两组患者的年龄、性别、临床症状和体征、实验室指标以及病情严重程度进行分析。结果　伴有基础疾病的患者(A组)占27 .5% (355 /1 291),其中40岁以上的患者占65. 6%;而无基础疾病的患者(B组)占72. 5% (936 /1 291),其中40岁以下的患者占74. 6%。本组基础疾病以高血压、糖尿病和冠心病最常见。A组患者的临床症状和体征的发生率明显高于B组(P<0 .05)。在病程中A组患者的淋巴细胞和血小板以及血生化等指标异常率明显高于B组患者(P<0. 05)。A组SARS患者的重症发生率为52. 7% (187 /355)。结论　合并基础疾病的SARS患者其临床表现较重,易出现实验室指标的异常,重症发生率高。

Clinical features of severe acute respiratory syndrome patients complicated with underlying diseases

Objective To investigate the clinical and laborat ory characteristics of SARS patients complicated with underlying diseases.Methods 1 291 patients clinically diagnosed as SARS defined by Chinese Ministry of Health reported in Beijing area were analyzed retrospect ively in our study.Results In 1 291 cases,there were 27.5% patients with unde rlying diseases,in which,65.6% cases were 40 years or older.There were 72.5% p atients without underlying diseases,in which,74.6% cases were 40 years or young er.Hypertension,diabetes mellitus and coronary heart disease were the most commo n underlying diseases of SARS.Clinical symptoms and signs of SARS in underlying diseases group were more than that in without underlying diseases group (P<0 .05).The abnormal levels of lymphocyte,thrombocyte,serum biochemical criterion of SARS in underlying diseases group were significantly higher than that in with out underlying diseases group(P<0.05).Morbidity of severe cases of SARS in underlying diseases group was 52.7%.Conclusion The SARS patients complicated with underlying diseas es are more severe at clinical and laboratory characteristics,and incidence of s evere cases of SARS in underlying diseases is higher.
