基于文献计量分析的我国DRG实施现状及效果研究

目的梳理我国疾病诊断相关分组(DRG)在各试点地区的实施情况。方法系统检索知网、Pub Med数据库2015-2019年发表文献,通过纳排标准筛选文献。采用文献计量分析对纳入的文献进行特征分析。结果共纳入中文文献12篇,英文文献1篇,其中2017年最多,其次为2018年。在考核DRG的指标中,使用频数较高的几项指标为病例组合指数(CMI)、平均住院日、时间消耗指数和费用消耗指数。试点地区总体呈现了医疗费用减少、医疗服务效率提高的趋势,但仍然还有许多问题如统一编码、准确分组以及医疗机构内部管理等亟待我们解决。结论加强全国统一DRG临床编码的编写,同时建立专门负责DRG的机构,培养专门的编码人才,革新医院管理方法与观念。

Status Quo and Effect of the Implementation of DRG in China Based on Bibliometric Analysis

Objective To sort out the implementation of DRG in China.Methods The system searched the knowledge network and Pub Med database from 2015 to 2019,and published the literature through the standard of exclusion.Characterization of the included literature was performed using bibliometric analysis.Results A total of 12 Chinese literatures and 1 English literature are included,with the most in 2017,followed by 2018.Among the indicators for assessing DRG,several indicators with higher frequency are the case combination index(CMI),the average hospitalization day,the time consumption index,and the cost consumption index.The pilot area generally shows a trend of reducing medical expenses and improving the efficiency of medical services,but there are still many problems,such as unified coding,accurate grouping,and internal management of medical institutions,which are urgently needed to be solved.Conclusion The preparation of national unified DRGs clinical codes should be strengthened,institutions dedicated to DRGs be established,specialized coding talents be trained,and hospital management methods and concepts be innovated.
