血管生长抑制因子对兔耳增生性瘢痕的作用及意义

摘    要:
目的通过观察血管生长抑制因子(内皮抑素)对兔耳增生性瘢痕的新生毛细血管及血管内皮细胞的影响,探讨内皮抑素防治病理性瘢痕的可能性。方法选取新西兰大耳兔16只,在兔耳腹侧面制作直径为6mm全层皮肤缺损创面,每耳4孔,共计128孔,建立增生性瘢痕模型,随机分为内皮抑素治疗组、生理盐水对照组;观察瘢痕变化。结果经内皮抑素局部注射治疗后的瘢痕持续时间缩短,同期瘢痕面积小于对照组(P<0.01)。治疗组病理组织切片光镜下同期微血管数少于对照组(P<0.01)。治疗组电镜下发现血管内皮细胞凋亡,细胞核核膜不完整、固缩,染色质分布不均、边聚,线粒体肿胀变性,嵴突破坏;对照组内皮细胞大多结构完整。结论内皮抑素可能通过抑制血管内皮细胞增殖及新生血管的形成,V诱发内皮细胞凋亡,起到一定防治瘢痕的作用,可能为临床治疗病理性瘢痕提供一条新的途径。

Abstract:
Objective To investigate the possibility of hyperplastic scars treated with endostatin,and to observe the effects on endothelial cells and microvessel of hyperplastic scars.Methods In total 16 rabbits,the wounds were created as follows:6 mm round wounds with total skin loses on the ventral side,4 holes for each ear,total 128 holes.The animals were divided into 2 groups:Endostatin treatment group and NS control group.The changes of the 2 groups were evaluated through light microscopy and electron microscopy.Results The area of scar in treatment group was obviously less than that in control group(P<0.01).Pathological sections showed that the counts of capillaries in the treatment group were obviously less than that of control group(P<0.01).Electron microscopy showed that that endothelial cell was apoptotic,nuclear membrane pycnosis,chromatin margination and irregular distribution,mitochondrial swelling and cristal destruction.The ultrastructures in control group were more intact than that in the treatment group.Conclusions Endostatin could prevent and treat the scar through the endothelial cell apoptosis,suppressing the hyperplasia of endothelial cells and the formation of the regenerated blood vessel,and it offers a new way for treating the hyperplastic scar clinically.
