年龄≤40岁青年人胃癌45例临床分析

目的：分析年龄≤40岁青年人胃癌的临床特征和病理特点。方法：回顾性分析2000年1月至2012年12月期间胃镜检查或手术并经病理证实的45例年龄≤40岁青年人胃癌的临床表现及病理特点。结果：女性发病比例（53．3％）比男性高；临床症状隐匿、不典型，上腹隐痛常见，肿瘤好发部位为胃窦、胃角，现有常规诊断方法对年龄≤40岁青年人胃癌敏感性差；以恶性程度高的低分化腺癌和印戒细胞癌为主。结论：年龄≤40岁青年人胃癌发病隐匿、进展快、恶性度高，应尽早胃镜检查、多块活检并适时重复检查，以提高早期发现率或减少漏诊率。

Clinical analysis of 45 cases of youth with gastric cancer

Objective： To investigate the clinical manifestation and pathological characteristics of gastric carcinoma in young adults ~〈 40 years old. Methods： A retrospective analysis for clinical manifestation and pathological characteristics in 45 cases of young with gastric carcinoma confirmed by endoscopy, operation and pathology was performed in our hospital from January 2000 to December 2012. Results： The ratio of disease development was higher in female than in male. Clinical symptoms were concealed and atypical with common upper abdominal dull pain. Tumor positions were generally found in gastric antrum and stomach angle. The conventional diagnostic methods available now are insensitive to the diagnosis of young patients with gastric cancer, of which the most are poorly differentiated and signet ring cell carcinoma with high malignancy. Conclusion： Great attention should be paid to the early gastric carcinoma in young adults, to whom a gastroscopy examination should be performed as soon as possible. A repeat endoscopic evaluation and biopsy may be appropriate in order to obtain early diagnosis and low rate of misdiagnosis.
