文拉法辛对氟西汀治疗无效的抑郁症患者的疗效分析

目的研究文拉法辛对氟西汀治疗无效的抑郁症患者的效果。方法对经氟西汀治疗8周无明显疗效的34例抑郁症患者以文拉法辛治疗6周（研究组）,剂量为（100±20）mg/d。在文拉法辛治疗前及治疗后第1,2,4,6周末评定汉密顿抑郁量表（17项,HAMD）,并进行临床疗效评定,监测血压,记录不良反应。选择同期门诊34例年龄、诊断与研究组相匹配的患者作为对照组,给予文拉法辛治疗6周,剂量为（99±25）mg/d。结果两组患者的HAMD评分均从第1周末起明显下降（P〈0.05或P〈0.01）,且一直持续至治疗第6周末。研究组治疗第6周末的有效率达68%（23/34）,临床治愈率达53%（18/34）;对照组分别为77%（26/34）和59%（20/34）。两组间的差异无统计学意义（P〉0.05）。研究组口干、恶心、呕吐的发生率高于对照组,差异有统计学意义（P〈0.05）。结论文拉法辛对氟西汀治疗8周无明显疗效的抑郁症患者有较好的疗效。

Analysis on Efficacy of Venlafaxine in Treating Fluoxetine-resistant Depression

Objective To study the efficacy of Venlafaxine on treating Fluoxetine-resistant depression.Methods A total of 34 cases of Fluoxetine-resistant depression were treated with Venlafaxine for 6 weeks（study group）,the dosage was（100±20）mg/day.Before treatment and after 1st,2nd,4th,6th weeks of treatment,the extend of depression was evaluated with Hamilton Depression Scale（17 items,HAMD）,and the efficacy was assessed,blood hypertension was monitored and side-effect was recorded.In the mean time,34 outpatients matched up with the patients of the study group were selected as the control group,and treated with Vnlafaxine for 6 weeks,the dosage was（99±25）mg/day.Results The HAMD scores of both groups decreased significantly from 1st weekend to 6th weekend（P0.05 or P0.01）.At the 6th weekend,the effective rate of the study group was 68%（23/34）,and clinical cure rate was 53%（18/34）;the effective rate and clinical cure rate of the control group was 77% and 59% respectively.There was no significant difference between two groups（P0.05） The incidence of dry mouth,nausea and vomiting of the study group was higher than the control group,and there were significant differences（P0.05）.Conclusion Venlafaxine has better efficacy in treating Fluoxetine-resistant depression.
