超声造影对前列腺癌的诊断价值探讨

目的探讨经直肠超声造影对前列腺癌的诊断价值。方法对所有受检者行超声造影,观察造影全过程。用ACQ软件对病灶组织造影参数进行分析比较,并对可疑区域实施穿刺。结果 62例患者经病理诊断为前列腺增生41例,前列腺癌21例。健康志愿者及前列腺增生患者造影剂灌注过程为内腺包膜及尿道部位先增强,紧接着是内腺、外腺包膜、外腺;消退的顺序则相反。前列腺癌的造影剂灌注为整体快速不均匀增强,内外腺分界不清。前列腺癌病灶的灌注参数始增时间为（11.5±2.65）s,小于良性病变的（14.4±3.29）s;达峰时间为（30.53±7.35）s,小于良性病变的（35.86±6.52）s;峰值强度为（-37.5±9.16）dB,大于良性病变的（-48.3±3.28）dB,均P〈0.05。结论良恶性前列腺病变经直肠超声造影的灌注方式有一定的规律性;ACQ分析软件可用来评估前列腺恶性病变的微血管生成情况。两者联合应用对前列腺癌的诊断具有一定的价值。

Evaluation of contrast-enhanced transrectal ultrasound in diagnosis of prostatic cancer

Objective To study the evaluation of contrast-enhanced transrectal ultrasound in diagnosis of prostatic cancer.Methods All persons were observed by using contrast-enhanced ultrasound.ACQ technique was used to analyze the parameters of all the persons’tissue.The puncture was implemented at the region with contrast suspicious.Results Sixty-two patients were diagnosed according to pathological results.Forty-one patients got prostatic hyperplasia and 21 patients got prostatic cancer.In the normal group and the group of benign prostatic hyperplasia,the enhanced perfusion appeared first in the inner gland capsule and urethra,then occurred in the inner gland,and the last in the envelope of the external gland;However,the regression order was opposite.In the cases of diffuse prostate cancer,the perfusion was enhanced as a whole,and it was rapid and uneven;The boundary between the inner and external gland was not so clear.The difference including the time of initial enhancement,peak enhancement and the intensity of peak enhancement between the groups had statistical significance （P0.05）.Conclusion The perfusion of prostate lesions through transrectal ultrasound contrast has some kinds of regularity.The ACQ software can be used to evaluate the capillary formation of prostate cancer.The associated application of the two methods can serve as the effective ways for diagnosis of the prostatic cancer.
