腰椎骨折与腰椎退变患者经腰椎后路植骨融合内固定后2年邻近节段退变的影像学表现

目的分析腰椎骨折与腰椎退变患者经腰椎后路植骨融合内固定后2年邻近节段退变(ASD)的影像学表现。方法选取2010年1月至2013年1月本院收治的36例腰椎骨折患者和30例腰椎退变患者分别设为骨折组和退变组。骨折组给予后路减压植骨融合内固定术治疗,退变组给予后路椎间融合术治疗。术后随访2年,并依据DR和改良Pfirrmann腰椎间盘突出MRI分级变化评估两组的ASD发生率。结果术后随访2年,骨折组的ASD发生率低于退变组,差异具有统计学意义(P<0.05)。结论腰椎退变患者内固定手术后对ASD影响大于骨折患者。

Imaging features of adjacent segment degeneration in patients with lumbar fracture and lumbar degeneration after 2 years of posterior fusion and internal fixation

Objective To analyze the imaging features of adjacent segment degeneration(ASD) in patients with lumbar fracture and lumbar degeneration after 2 years of posterior fusion and internal fixation.Methods From January 2010 to January 2013,36 patients with lumbar fracture and 30 patients with lumbar degeneration admitted in our hospital were selected as fracture group and degeneration group respectively.The fracture group was treated with posterior decompression and fusion and internal fixation,and the degeneration group was treated with posterior interbody fusion.The patients were followed up for 2 years,and the incidence of ASD between the two groups was evaluated according to DR and MRI grading changes of modified Pfirrmann lumbar disc herniation.Results During 2-year follow-up,the incidence of ASD in the fracture group was lower than that in the degeneration group,and the difference was statistically significant(P<0.05).Conclusion The effect of internal fixation on ASD in patients with lumbar degeneration was greater than that in patients with fracture.
