磁共振扫描在膝关节剥脱性骨软骨炎诊断中的应用价值

目的探讨MR多序列扫描对膝关节剥脱性骨软骨炎的诊断及分期应用价值,并关节镜检查结果对照分析。方法对2005年6月～2007年6月期间的38例临床症状明显而普通X线未能明确诊断或疑似膝关节剥脱性骨软骨炎的病例采用MR多序列扫描,分析其MRI表现特点,并对有关节镜检查结果的患者进行对照分析。结果38例疑似病例中,右膝24例,左膝8例,双膝6例,MR诊断为阳性的36例,受累部位中,股骨内侧髁26处,股骨外侧髁14处,髌骨3处,胫骨平台5处,合共47个病灶,分析各例MR表现特征,对病例进行分级,其中0级的2例,1级的10例,2级的12例,3级的9例,4级的5例,对18例进行了关节镜检查的患者作病例对照,与各级MRI表现所见均一致。结论MR多序列扫描可清晰显示膝关节剥脱性骨软骨炎中软骨信号改变、碎裂程度及移位情况,与关节镜结果一致,对临床诊断及治疗膝关节剥脱性骨软骨炎具有重要价值。

The applied value of MR in the diagnosis of osteochondritis dissecans in the knee

Objectives To summarize the applied value of multi-sequenced MR scan in the diagnosis and the phase of Osteochondritis Dissecans in the knee.Methods apply multi-sequenced MR to scan 38 patients who had obvious clinical symptoms,doubtful to Osteochon-dritis Dissecans in the knee at X-ray results,then analyze MRI characteristics,compared to the outcomes by arthroscopy.Results 36 cases was positive by multi-sequenced MR scan. Of which 26 cases are in the medial femoral condyle ,14 cases in the lateral femoral condyle,3 cases in the patella,the other 5 cases in the tibial platform,47 foci altogether.Classify all the cases according to the MRI characteristics,2 cases in Grade-0,10 cases in Grade-1,12 cases in Grade-2,9 cases in Grade-3,5 cases in Grade-4; which are consistent with 18 cases by arthroscopy.Conclusions Multi-sequenced MR scan that can clearly show the change of the cartilaginous sign,the fragmental extent and the shifted condition of Osteochondritis Dissecans in the knee plays an important role in diagnosing and treating the Osteochondritis dissecans.
