神经内科临床药师工作模式探索

神经内科患者以老年人居多，存在肝肾功能减退等现象，且多伴有高血压、糖尿病等慢性疾病，联合用药多，依从性差，需加强对此类患者的药学监护。临床药师通过协助医师制定治疗方案、干预不合理用药、传达药物警戒信息、监测药物不良反应、为医护人员提供药物咨询、加强患者用药教育、建立药历及药学路径表等方式，开展药学服务工作，探索临床药师工作模式。

Exploration of clinical pharmacist working mode in the department of neurology

The patients in neurology department mostly are elderly people, who are with liver or kidney dysfunction and combined with other chronic diseases such as hypertension, diabetes and so on. More pharmaceutical cares should be paid attention to the patients because of their more combination use of drugs and poor compliance. The clinical pharmacist carried out the work of pharmacy services and explored the working mode by assisting doctor to make treatment plan, intervening irrational drug use, conveying pharmacovigilance information, monitoring adverse drug reaction, providing medication consultation for medical staffs, conducting patient education, establishing drug history documents and pharmaceutical path tables, etc. Clinical pharmacist has become a good medical assistant staff and plays a role in the medical team.
