经皮腔内血管成形术加内支架置入治疗肝静脉膜状阻塞所致布-加综合征

目的　探讨肝中静脉阻塞的处理方法。方法　选择肝静脉膜状阻塞 (MOHV)所致肝静脉阻塞患者 ,在彩色多普勒引导下应用PTHV方法穿刺肝中静脉 ,然后在DSA引导下行肝静脉腔内血管成形术 (PTA)和血管内支架置入。支架植入时 ,在下腔静脉肝中静脉开口处置放球囊 ,以防止支架移位。结果　应用该方法手术顺利 ,术后随访 3年 ,效果良好。结论　PTA加内支架置入是治疗MOHV所致布 -加综合征安全、可靠的有效方法

Percutaneous transluminal angioplasty and intraluminal stent for treatment of Bubb-Chiari syndrome induced by menbransus obstruction of the hepatic veins

Objective To investigate the therapy of Bubb-Chiari syndrome(BCS) due to membranous obstruction of the hepatic veins(MOHV).Method One patient with membranous obstruction of middle hepatic vein was treated with percutaneous transhepatic hepatovenography(PTHV) under ultrasound guidance, and then percutaneous transluminal angioplasty(PTA) and stent placement of the hepatic vein were undergone under DSA guidance. Balloon was placed at orifice of middle hepatic vein in the inferior vena cava(IVC) to avoid movement of stent in hepatic vein.Result PTA and stent placement were successful,and his symptoms and signs disappeared during a follow-up period of 3 years.Conclusion The interventional therapy is safe and effective to BCS due to MOHV.
