大黄蛰虫方合逍遥方加减对原发性肝细胞癌 TACE术后综合征影响

目的 探讨大黄蛰虫方合逍遥方加减对原发性肝细胞癌TACE术后综合征影响及临床意义。方法 选取2016年10月~2017年8月来我院肿瘤科30例及邯郸中心医院介入科30例就诊的Ⅱ~Ⅲ期不能手术的原发性肝细胞癌患者，随机分为治疗组和对照组，每组30例。对照组采取单纯经皮肝动脉化疗栓塞术治疗，治疗组在对照组基础上联合大黄蛰虫方合逍遥方加减治疗。观察两组患者治疗后不良反应发生率以及肝功能变化情况。结果 治疗组恶心、呕吐、发热、腹痛发生率分别为16.67%、13.33%、26.67%、20.00%，对照组恶心、呕吐、发热、腹痛发生率分别为33.33%、30.00%、40.00%、36.67%，治疗组术后综合征发生比例少于对照组，差异有统计学意义（P＜0.05）。观察术后3 d、7 d、14 d肝功能中谷丙转氨酶ALT值治疗组指标低于对照组，差异有统计学意义（P＜0.05）。结论 大黄蛰虫方合逍遥方加减联合经皮肝动脉化疗栓塞术，疗效显著，可以明显降低原发性肝细胞癌TACE术后综合征发生率。

Effect of Rhubarb Aphid Fanghe Xiaoyaofang Decoction on Postoperative TACE Syndrome of Primary Hepatocellular Carcinoma

Abstract:Objective To investigate the effect and clinical significance ofRhubarb Aphid combined with Xiaoyaofang Decoction on postoperative TACE syndrome in primary hepatocellular carcinoma.Methods From October 2016 to August 2017,30 patients with oncology in our hospital and 30 patients with interventional department in the Central Hospital were treated with stage II~III inoperable primary hepatocellular carcinoma.They were randomly divided into treatment group and control group,30 cases in each group.The control group was treated with simple percutaneous transhepatic arterial chemoembolization.The treatment group was treated with the combination of Rhubarb Aphid Fanghe Xiaoyaofang Decoction.The incidence of adverse reactions and changes in liver function after treatment were observed.Results The incidences of nausea,vomiting,fever and abdominal pain in the treatment group were 16.67%,13.33%,26.67%,and 20.00%,respectively.The incidences of nausea,vomiting,fever,and abdominal pain in the control group were 33.33%,30.00%,40.00%,and 36.67%,respectively.The incidence of postoperative syndrome in the treatment group was lower than that in the control group,and the difference was statistically significant (P<0.05).The indexes of alanine aminotransferase ALT in the liver function were lower than those in the control group at 3,7 and 14d after operation,the difference was statistically significant(P<0.05).Conclusion Rhubarb Aphid combined with Fanghe Xiaoyaofang Decoction combined with percutaneous transhepatic arterial chemoembolization has significant curative effect and can significantly reduce the incidence of postoperative TACE syndrome in primary hepatocellular carcinoma.
