N-CPAP模式和NIPPV对呼吸窘迫综合征早产儿心功能的影响分析

目的 分析N-CPAP模式和同步间歇正压通气（NIPPV）对呼吸窘迫综合征（NRDS）早产儿心功能的影响。方法 选取2012年6月~2015年5月在我院出生的90例NRDS患儿，随机分为对照组和实验组各45例。对照组采用N-CPAP（经鼻持续气道正压方法）治疗，实验组采用NIPPV（同步间歇正压通气方法）治疗，对比两组患儿治疗前后氧合指数、血气变化、并发症及总成效。结果 实验组治疗1 h后患儿血气变化和氧合指数明显升高，均大于对照组，实验组在总体疗效（77.78%）比较上也明显高于对照组（55.56%），差异具有统计学意义（P＜0.05）；两组均未发生并发症。结论 NRDS患儿中用NIPPV进行初始通气治疗总体效果明显优于N-CPAP，值得临床推广。

Effect of N-CPAP Mode and NPIPPV on Cardiac Function in Preterm Infants with Respiratory Distress Syndrome

Abstract:Objective To analyze the N-CPAP model and the synchronized intermittent positive pressure ventilation(NIPPV)on respiratory distress syndrome(NRDS)on heart function in premature infants.Methods From June 2012~2015 year in May 90 cases of NRDS children born in our hospital, were randomly divided into control group and experimental group with 45 cases in each group.The control group was treated with N-CPAP(nasal continuous positive airway pressure method) treatment,the experimental group adopted NIPPV(SIPPV method)treatment,oxygenation index,blood gas changes,complications and total effect were compared between two groups before and after treatment.Results After 1 h of treatment,the changes of blood gas and oxygenation index in the experimental group were significantly higher than those in the control group.The experimental group was significantly higher than the control group(77.78%)compared with the control group(55.56%),the difference was statistically significant significance(P<0.05);no complications occurred in both groups.Conclusion The overall effect of NIPPV for initial ventilation in NRDS children is better than that of N-CPAP,which is worthy of clinical promotion.
