老年冠心病患者窦性心率震荡现象的临床价值分析

目的探讨老年冠心病患者窦性心率震荡的临床意义。方法选取符合条件的116例老年冠心病患者和120例有室早的健康体检者经冠状动脉造影证实,进行24h动态心电图监测,并对其室早后的震荡初始（TO）和震荡斜率（TS）及其平均值进行测量对比分析。结果老年冠心病组室早后TO值高于对照组;TS值低于对照组,差异有非常显著意义（P〈0.01）。冠心病组中TO和TS异常的阳性率分别为44%和31.9%;对照组TO和TS异常的阳性率分别为20.8%和12.5%。两组阳性率差异有统计学意义（P〈0.05）。冠心病组TO和TS均异常者27.6%,对照组TO和TS均异常者9.2%,两组比较差异有显著意义（P〈0.01）。TO值与冠状动脉病变支数及恶性心律失常发生率呈正相关（r=0.470,P〈0.05）,TS值与冠状动脉病变支数及恶性心律失常发生率呈负相关（r=0.611,P〈0.05）。结论老年冠心病患者TO、TS值均有不同程度异常,异常程度随病变严重程度而增加,且TO、TS值均异常时心源性猝死风险增高。及时观察老年冠心病患者的TO和TS值对危险分层及预后判断均具有重要临床意义。

Sinus heart rate turbulence in elderly patients with coronary heart disease clinical value analysis

Objective To study the Clinical significance of sinus heart rate turbulence in patients with coronary heart disease Methods Select early to comply with the conditions of 116cases of elderly patients with coronary heart disease and 120cases of room healthy,Confirmed by coronary angiography,After 24-hour Holter monitoring,and ventricular premature turbulence onset（TO）and turbulence slope（TS）and the average measured comparative analysis.Results TO value in the elderly CHD group PVCs is higher than that of the control group;the TS value is lower than the control group,the difference was significant（P0.01）.For the Coronary heart disease group,the abnormal positive rates of TO and TS are 44%and 31.9%.For the control group,the positive rate of TO and TS abnormal are 20.8%and 12.5%.Two sets of positive rate difference was statistically significant（P0.05）The Coronary heart disease group TO and TS were abnormal by 27.6%,the control group TO and TS were 9.2%of the abnormal,There is Significant difference between the two groups.TO value and the number of coronary lesions and malignant arrhythmia incidence was positively correlated（r=0.470,P0.05）,TS value with coronary lesion count and malignant arrhythmia incidence was negatively correlated（r=0.611,P0.05）.Conclusion Patients with coronary heart disease,TO,TS value have varying degrees of abnormal degree of abnormality increases with the severity of the disease,and TO and TS values were abnormal sudden cardiac death rate increased.TO and TS values timely observation of elderly patients with coronary heart disease risk stratification and prognosis has important clinical significance.
