手足口病患者体液免疫与病情转归的相关性分析

目的：探讨手足口病患者血清免疫球蛋白及补体与病情转归的关系。方法：研究纳入102例手足口病患儿，按病情转归分成三组：合并神经源性肺水肿的重症手足口病组（Ⅰ组，24例）、未合并神经源性肺水肿的重症手足口病组（Ⅱ组，39例）、轻症手足口病组（Ⅲ组，63例），另取同期住院的畸形矫治患儿为正常对照组（Ⅳ组，51例）。比较各组间血清免疫球蛋白IgA、IgG、IgM及补体C3、C4水平。结果：均数比较，合并肺水肿的重症手足口病患者血清IgG升高（15.29±11.77 g/L），未合并肺水肿的重症手足口病患者血清IgA水平升高(0.73±0.54 g/L)，各组手足口病患儿血清C3、C4水平较正常对照组升高，差异均有统计学意义（P<0.05）。结论：手足口病患者存在体液免疫功能紊乱，血清IgG升高可能是手足口病患者出现肺水肿的机制之一。

Correlation between the outcome and humoral immunity in patients with hand foot and mouth disease

Objective: To study the relationship between the outcome and immunoglobulin and complement in the hand foot and mouth disease patients． Methods: 102 cases of hand, foot and mouth disease children are Included. They were divided into three groups according to disease prognosis. IgA、IgG、IgM and complement C3、C4 （in blood serum） were evaluated and evaluated between different group． Results: The level of serum IgG in the group of severe hand, foot and mouth disease infants whose with pulmonary edema were greatly increased than other groups. The level of C3 and C4 were greatly increased in the hand foot and mouth disease groups than the control groups. Conclusion: The patient of Hand, foot and mouth disease has dysfunction of humoral immune, serum IgG increased may be one of the mechanisms of pulmonary edema in hand, foot and mouth disease.
