阿托伐他汀在治疗扩张型心肌病心力衰竭中的应用

目的探讨他汀类药物在非缺血性心力衰竭的治疗中的应用。方法将30例特发性扩张型心肌病患者随机分为阿托伐他汀组（n=15）和安慰剂组（n=15）。阿托伐他汀组在常规治疗的基础上，加用阿托伐他汀20need。结果治疗16周后，阿托伐他汀组较安慰剂组血浆总胆固醇和低密度脂蛋白水平降低，心功能改善，左室射血分数提高，为（39．11±3．45）％对（34．23±3．14）％，P〈0．01。血浆肿瘤坏死因子-α（TNF—α）和C-反应蛋白（CRP）水平在阿托伐他汀组显著降低（P〈0．01）。结论阿托伐他汀可以显著改善扩张型心肌病心袁患者的心功能和血管内皮功能，提示他汀类药物可用于治疗非缺血性心力衰竭。

Atorvastatin in Treatment of Dilated Cardiomyopathy

Objective Chronic heartfailure(CHF) is associated with inflammation.The 3-hydroxy-3-methylgutary-CoA(HMG-CoA) inhibiters,statins,exert anti-inflammation and vascular protective effects,we hypothesized that stain therapy has beneficial effects in patients with nonischemic heart failure.Methods thirty six patients with nonischemic dilated cardiomyopathy were randomly divided into two groups.One group(n=15) received statin(atorvastatin),the other group(n=19) reived placebo.The intial dose of atorvastatin was 20 mg/d.Results After 16 weeks,the group receiving atorvastatin exhibited a reduction in plasma chlolesterol and LDL compared to placebo group.NYHA cardiac function class in atorvastatin gtoup was decreased as compared with the placebo group(P<0.05) and left ventricular eject fraction(LVEF) was inproved in patients treated with atorvastatin but not in placebo group,(39.11±3.45)% vs(34.23±3.14)%.The plasma concentration of TNF-α and CPR were significant decreased in atorvastatin group(P<0.01).Conclusion Atorvastatin can improve cardic function and endothelium function in patients with nonischemic cardiomyopathy.It suggests that statin may have therapeutic benefits in nonischemic heart failure.
