超声消融联合干细胞移植治疗老年动脉硬化性闭塞症

目的 观察血管内超声消融联合自体骨髓干细胞移植治疗老年性下肢动脉硬化性闭塞症的疗效。方法 22例老年性下肢动脉硬化闭塞症的患者，男13例，女9例。平均年龄65岁。手术：首先抽取自体骨髓干细胞260mL，分离出单个核细胞悬浊液40mL。采用经皮股动脉穿刺或显露切开插入超声消融导管，在动脉造影监视下进行血管内超声消融。而后将干细胞悬浊液注射小腿缺血肌肉内局部。结果 22例患者静息痛缓解，跛行距离增加，冷、凉感觉得到改善。结论 血管内超声消融联合自体骨髓干细胞移植治疗老年动脉硬化性闭塞症，是一种有效的方法。

Ultrasound Ablation and Autologous Transplantation of Bone Marrow Mononuclear Cells for Treating Elderly Patients with Arteriosclerosis Obliterans

Objective To investigate the efficacy of ultrasound ablation and autologous transplantation of bone marrow mononuclear cells in treatment of elderly patients with arteriosclerosis obliterans.Methods 22 patients with arteriosclerosis obliterans were treated with ultrasound ablation and autologous transplantation of bone marrow mononuclear cells.Through percutaneous approach or surgical approach the ultrasound ablation catheter was inserted into occluded arterial segments.After ablation performed,autologous bone marrow mononuclear cells were directly injected into the skeletal muscles of the ischemic crura.Results No complications were found in this group.Postopertaively rest pain of all the patients got significient improvement.The skin turned warm in all legs.Conclusion Ultrasound ablation and autologous transplantation of bone marrow mononuclear cells could be simple,safe and effective method for treating patients with diabetic feet.Although its long term effect need to follow up and in more patients,an alternative choice to treat these patients is found expect amputation.
