妊娠期糖尿病患者胎儿脐血血清中SHBG水平的变化及意义

摘    要:
目的 检测妊娠期糖尿病(GDM)患者其新生儿脐血血清中性激素结合球蛋白(SHBG)、血糖、胰岛素及IR的水平。探讨其在新生儿脐血中的变化规律及对新生儿生长发育的影响。方法 采用酶联免疫吸附双抗体夹心法(ELISA)测定22例GDM患者和21例正常妊娠妇女新生儿血清中SHBG的浓度,用放免电化学发光法检测血清中的胰岛素(INS)浓度。结果 GDM组实验组脐血中SHBG浓度、血糖水平均明显低于正常对照组,差异显著(P〈0.01);空腹胰岛素浓度明显高于正常对照组,差异显著(P〈0.01),IR在两组中无明显差异(P〉0.05)。SHBG与体重及脐血FIns浓度均呈负相关。结论 SHBG与肥胖关系非常密切,它可能是早期肥胖及DM发生、发展的高危因素,也可能成为预测新生儿发展为肥胖的一个重要指标。

Abstract:
Objective:To investigate the level of sex hormone binding globulin,FBG,Fins and IR in cord blood serum with Gestational diabetes mellitus,in order to find their effects to newborns.Methods:Enzyme linked immunosorbent assay(ELISA)was used to determine serum SHBG levels in 22 newborns with Gestational diabetes mellitus and 21 with normal pregnant women.Results:The SHBG and FBG levels of cord blood in GDM groups were significantly lower than that in the control group(P<0.01);and the levels of FIns were significantly higher than those in the control group(P<0.01).IR in two groups were no remarkable difference.There was a negative correlation between SHBG and body weight of newborns,also between SHBG and FIns.Conclusion:There is a correlation between SHBG and fat.It may be a high risk of early stage of fat and DR.SHBG may also be a index to judge the newborn to be fat in the future.
