股静脉穿刺留置在ICU微循环障碍患者抢救中的应用

目的探讨股静脉穿刺留置在ICU微循环障碍患者抢救过程中的作用。方法将92例微循环障碍患者按入院时间的先后分为研究组（股静脉穿刺留置）和对照组（颈内外静脉或周围静脉穿刺）各46例，观察两组患者生命体征变化、不良反应发生情况、一次穿刺成功率及人均穿刺所需时间。结果两组患者穿刺前后心率、血压等生命体征变化均无统计学意义（P〉0．05）；研究组发生针头脱落、皮下淤血、针头阻塞及深浅静脉炎的患者均少于对照组（P〈0．05或0．01）；股静脉穿刺组一次穿刺成功率（97．8％）高于对照组（80．4％），两组人均穿刺所需时间无统计学意义（P〉0．05）。结论股静脉穿刺留置能达到及时抢救和迅速补液的效果，是微循环障碍患者抢救过程中建立静脉通道的一条安全、有效的途径。

Clinical Application of Indwelling Needle of Femoral Vein Puncture in Rescuing Patients with Microcirculation Disturbance

Objective To investigate the role of indwelling needle of femoral vein puncture in rescuing the patients with microcirculation disturbance. Methods Ninety-two patients with microcirculation disturbance were equally divided into the observation group （indwelling needle of femoral vein puncture）and the control group（indwelled needle in external jugular vein ＆ peripheral vein puncture） according to the admission sequence. The changes in vital signs, occurrence of side effects and first-time success rate of the puncture and average puncture time in the patients of both groups were observed. Results There were no significant changes in the vital signs of cardiac rate and blood pressure before and after the puncture（P〉0.05）. The occurrence rates of detached needle,subcutaneous ecchymosis, needle obstruction and deep ＆ superficial phlebitis were lower in the observation group than those in the control group（P〈0.05 or 0.01）. The first-time success rate of femoral vein puncture of the observation group（97.8%）was superior to that（80.4%）of the control group. No significance was observed on the average puncture time between the two groups （P〉0.05）. Conclusion Indwelling needle of femoral vein puncture can safely and effectively fulfill the requirements of timely rescuing and rapid fluid infusion in rescuing patients with microcirculation disturbance.
