双侧额叶脑挫裂伤致脑中心疝的临床诊治分析

目的 分析双侧额叶脑挫裂伤致脑中心疝的临床特点及手术时机，探讨提高临床诊疗效果的方法。方法 回顾性分析62例双侧额叶脑挫裂伤致脑中心疝手术患者的临床症状、影像学特征及手术时机，根据手术时机将患者分为间脑期组34例和非间脑期组28例，比较两组患者的预后。结果 两组患者预后比较，差异有统计学意义（Z=-3.154，P=0.002），间脑期组好于非间脑期组。结论 早期识别脑中心疝间脑期并积极开颅手术，能够提高患者的抢救成功率及改善预后。

Clinical diagnosis and treatment of central brain hernia caused by bilateral frontal lobe contusion and laceration

Objective To analyze the clinical characteristics of central brain hernia caused by bilateral frontal lobe contusion and laceration and timing of surgery, and to explore approaches for improving clinical outcomes.Methods A retrospective study was performed on clinical symptoms, imaging features, and timing of surgery in 62 patients with central brain hernia caused by bilateral frontal lobe contusion and laceration. According to the timing of surgery, the patients were divided into diencephalon group (n=34) and non-diencephalon group (n=28). The prognosis was compared between the two groups.Results The diencephalon group had a significantly better prognosis than the non-diencephalon group (Z=-3.154, P=0.002).Conclusions Early recognition of diencephalon and proactive craniotomy could promote the success rate of clinical rescue and improve the prognosis in patients with central brain hernia.
