自攻型微型种植体支抗矫治上颌前突的临床研究

目的：探讨自攻型微型种植体支抗在严重上颌前突病例内收上颌前牙的临床疗效。方法：20例严重上颌前突拔牙矫治病例，在上颌第一磨牙与第二前磨牙之间颊侧植入微型种植体。通过头影测量分析，对矫治前后U1-NA距，U1-NA角，U1-Apog距分别进行配对t检验分析。通过上颌前牙内收量，来评判微型种植体作为强支抗的临床应用的稳定性。结果：20例平均6-8个月牙列排齐整平，10—12个月前突的前牙成功内收，拔牙间隙完全关闭，治疗过程有2枚种植体脱落，4枚种植体感染，34枚种植体整个过程保持稳定。结论：微型种植体可以提供足够稳定的强支抗内收前突前牙，矫治效果好，矫治疗程相对短。

A clinical study on the treatment of maxillary protrusion with the self-tapping screw.

Objective： To investigate clinical study on restraction of the maxillary protrusive anterior teeth during the severe maxillary protrusive cases with self-tapping screw. Method：Twenty patients with severe maxillary protrusion and tooth-extraction were collected. Forty micro-implant screws would be implanted between the first molars and the second pre- molars. The distance UI-NA,UI-APog and the angel UI-NA of preorthodontic and postorthodontic all were measured by cephalometric analysis. Pair t test was used to analyze all datas. It estimate the micro-implant screws clinical stability and applicable feasibility via the magnitudes of the maxillary anterior teeth＇s restraction. Result. Those twenty patients speeded six or eight months on average on alignment and leveling. Ten or twelve months on restraction of anterior teeth and closure of extraction space. In the procedure of treatment,2 microscrews had drop out,4 microscrews leaded infection, the rest all felt better, work well in the while treatment. The result between preorthodontic and postorthodontic was significant at P 〈0.05 calculations. Conclusion：The microimplant screws provide enough stable strong anchorage to restract anterior teeth and come true the ideal effect of orthodontical treatment. Moreover, the period of treatment go short relatively, patients would ac- cept it generally, no cooperate actively, and adapt it as possible as quickly.
