系统性红斑狼疮血脂结果分析和临床意义

目的探讨系统性红斑狼疮（sLE）血脂变化和临床意义。方法回顾性分析190例SLE患者血脂中甘油三酯（TG）和总胆固醇（TC）的特点。结果通过比较分析190例SLE患者中血脂水平的变化，发现使用激素组和LN组血脂水平升高明显，有统计学意义（P〈0．05），而LN患者中使用激素组和未用激素治疗组血脂水平无差别（P〉O．05），且血脂水平高低和24小时尿蛋白定量无相关性（P〉O．05）。结论SLE长期使用激素者和肾损害者血脂代谢明显异常，血脂异常是SLE患者心血管事件和无菌性股骨头坏死（ANFH）的危险因素，也参与肾损害。提示对SLE患者应建立个体血脂水平基线、进行血脂分析和长期血脂水平监测随访是必要的。

Analysis and Significance of Blood Lipids in Patients with Systemic Lupus Erythematosus

Objective To investigate the changes and the clinical significances of blood lipids in patients with systemic lupus erythematosus（SLE）.Methods Retrospective analysis was done onthe characteristics oftriglyceride （TG）and total cholesterol（TC） in 190 SLE patients.Results Through the analysis of blood lipids in 190 SLE patients, we found that the level of TG and total TC were elevated in SLE patients who had been treated with long terms glucocorticoid （GC） compared with patients without using of GC（P〈0.05）,we also observed lipids were higher in LN patients than that in ones who were simply SLE.However, there was no significant difference between LN patients who had been treated with GC and ones who hadn＇t been treated with GC（P〉0.05）,there was no correlation between 24 hours urine protein（24hUpro）and the levels of blood lipids （P〉0.05）.Conclusion Blood lipids abnormal- it）＇ was often found in SLE patients who had been treated with long terms GC and LN patients.It emerged as a risk factor of cardio-cerebral vessel incidents and of Avaseular Necrosis of the Femoral Head （ ANFH ）in SLE patients, and involved in the damages of kidney.It is suggested that it is necessary to establish the baseline of blood lipids, measure blood lipids and long term follow-up monitoring for SLE patients.
