家属疾病认知水平和疾病不确定感与缺血性脑卒中患者复发风险的关系研究

目的探讨家属疾病认知水平和疾病不确定感与缺血性脑卒中患者复发风险的关系。方法采用便利抽样法选取2013年1月—2014年2月深圳市3所二级甲等医院收治的389例缺血性脑卒中住院患者及其家属,根据患者发病次数将其分为对照组288例和复发组101例。采用自行编制的一般资料问卷、脑卒中知识问卷和疾病不确定感家属量表对患者家属进行调查。结果复发组患者家属疾病认知水平得分高于对照组患者家属,疾病不确定性维度、信息缺乏性维度及疾病不确定感总维度得分均低于对照组(P0.05);直线相关分析显示,家属疾病认知水平得分与缺血性脑卒中复发次数呈负相关(r=-0.245,P=0.014),疾病不确定感得分与缺血性脑卒中复发次数呈正相关(r=0.220,P=0.010)。结论家属疾病认知水平和疾病不确定感是缺血性脑卒中复发的影响因素,应采取措施提高家属疾病认知水平、降低其疾病不确定感,从而降低缺血性脑卒中患者复发风险。

Correlation between Disease Cognition and Uncertainty of Family Members and Recurrence Risk in Patients with Ische-mic Stroke

Objective To investigate the correlated between disease cognition and uncertainty of family members and recurrence risk in patients with ischemic stroke. Methods From January 2013 to February 2014,389 patients with ischemic stroke and their family members were chosen by convenience sampling method admitted in 3 secondary hospitals of Shenzhen,and they were divided into control group （ n =288 ） and recurrence group （ n =101 ） according to the recurrence of ischemic stroke. Then investigated the disease cognition and uncertainty of family members by self-made general information questionnaire, stroke knowledge questionnaire and disease uncertainty scale. Results The score of disease cognition of family members of re-currence group was higher than that of control group,while the scores of disease uncertainty dimension,lack of information di-mension and the total dimensions of disease uncertainty were lower than those of control group（P﹤0. 05）. Linear correlation a-nalysis showed that,the score of disease cognition of family members of recurrence group was negatively correlated with the recur-rence frequency of ischemic stroke（r= -0. 245,P=0. 014）,while the score of disease uncertainty was positively correlated with the recurrence frequency of ischemic stroke（r=0. 220,P=0. 010）. Conclusion Disease cognition and uncertainty of family members are influencing factors of recurrence of ischemic stroke,thus measures should be take to improve the disease cog-nition of family members,reduce disease uncertainty,to reduce the recurrence of ischemic stroke.
