脑性瘫痪伴痉挛痛患儿的生存质量调查研究

目的评估脑性瘫痪伴痉挛痛患儿的生存质量情况,为制定更恰当的治疗和干预措施提供依据。方法采用横断面病例对照研究设计。使用儿童生存质量测定量表(Pediatric Quality of Life Inventory TMversion 4.0,PedsQLTM4.0)中文版普适性核心量表(家长报告表)对54名脑性瘫痪伴痉挛痛患儿和100名正常儿童的生存质量进调查。结果脑性瘫痪伴痉挛痛患儿PedsQLTM4.0生存质量的四个亚量表及总分均比正常组得分低,差异有统计学意义(P0.05)。不同性别及不同年龄组(2~4、5~7、8~13岁)患儿的四个亚量表及总分比较,差异无统计学意义(P0.05);GMFCS分级的比较1~2级的生理功能、心理功能的得分比3~5级的明显高(P0.05),其它三个区域的得分差异无统计学意义(P0.05)。脑性瘫痪伴痉挛痛PedsQLTM4.0生存质量的的生理功能与学校功能的得分与GMFM-88的总分成正相关(r=0.478,0.413,P0.05)。结论脑性瘫痪伴痉挛痛患儿的生存质量较正常儿童的差,其生存质量与其脑性瘫痪的大运动功能有明显的相关性。

Investigation of the life quality of cerebral palsy children with spasm pain.

Objective To assess the life quality of the cerebral palsy (CP) children with spasm pain.Methods The method of cross-sectional case-control was used to investigate the life quality of 54 cerebral palsy children with spasm pain and 100 normal children by Pediatric Quality of Life Inventory TM Version 4.0 (PedsQLTM4.0).Results The scores (physical functioning,psychological functioning,emotional functioning,school functioning and total score)of study group were lower than those of the normal group (P<0.05);the scores had no statistical significances between the female and male;the scores of the five part had no statistical significances between 2~4 years old age group,5~7 years old age group and 8~13 years old age group(P>0.05);The scores of physical functioning and psychological functioning of 1~2 level of GMFCS (Gross Motor Function Classification System) were higher than those of the 3~5 level (P<0.05),the scores of other three part had no statistical significances (P>0.05);the score of physical functioning and school functioning had positive correlations with the total score of GMFM-88 (r=0.478,0.413,P<0.05).Conclusion The life quality of the cerebral palsy children with spasm pain is worse than the normal children,and the severity of their life quality has significant correlation with gross motor function.
