青黛凉血方联合阿维A及卡泊三醇治疗银屑病的疗效及对血清细胞因子的影响

目的观察青黛凉血方联合阿维A及卡泊三醇治疗银屑病的疗效及对血清细胞因子的影响,探讨其临床适用性。方法选择我院62例寻常性银屑病患者,随机分为试验组和对照组各31例,对照组患者给予阿维A口服联合卡泊三醇外涂治疗,试验组在此基础上联合青黛凉血方治疗。观察两组患者治疗前后银屑病皮损面积严重程度积分(PASI)情况,并统计疗效差异,同时分析治疗前后血清细胞因子白细胞介素-17(IL-17)、白细胞介素-6(IL-6)及肿瘤细胞坏死因子-(TNF-)的变化情况及用药期间不良反应情况。结果两组患者治疗前PASI评分比较,差异无统计学意义(P0.05),治疗后均显著降低(P0.05),且试验组患者治疗后PASI评分显著低于对照组,差异有统计学意义(P0.05),试验组患者治疗后的总有效率与对照组比较,差异有统计学意义(P0.05),两组患者治疗前IL-17、IL-6及TNF-比较,差异无统计学意义(P0.05),治疗后均显著下降(P0.05),且试验组患者治疗后低于对照组,差异有统计学意义(P0.05),两组患者均未出现严重不良反应。结论青黛凉血方联合阿维A及卡泊三醇治疗银屑病临床疗效佳,可能通过调节细胞因子达到治疗效果,用药安全,适合临床应用。

The Effects of Qingdai Liangxue Decoction Combined with Acitretin and Calcipotrio Ointmenon on Psoriasis and Its Influence on Serum Cytokines

Objective To observe the curative effect ofQingdai Liangxue decoction combined with acitretin and calcipotrio ointmenon in treatment of psoriasis and its influence on serum cytokines,and discuss its clinical applicability.Methods 62 cases of patients with psoriasis were randomly divided into control group and treatment group,31 cases in each group.Control group was treated with oral acitretin combined with calcipotrio ointmenon,treatment group received Qingdai Liangxue decoction on the basis of control group.To observe the psoriasis area severity scores (PASI) of both groups before and after treatment,statistic the curative effect differences,and analysis the serum cytokine interleukin 17 (IL-17),interleukin 6 (IL-6) and tumor necrosis factor alpha (TNF-alpha) changes and adverse reactions during treatment.Results There was no significant difference between the two groups in the PASI score before treatment (P＞0.05),which decreased significantly after treatment (P＜0.05),and the score of in treatment group was significantly lower than that in control group (P＜0.05).The total effective rate in treatment group was higher than that in control group,the difference was significant(P＜0.05).There were no statistically significant difference of IL-17,IL-6 and TNF-α of the two groups efore treatment (P＞0.05),and significantly decreased after treatment (P＜0.05),treatment group was lower than that of control group,the difference was statistically significant (P＜0.05).There were no serious adverse reactions in the two groups.Conclusions Qingdai Liangxue decoction combined with acitretin and calcipotrio ointmenon is effective in treatment of psoriasis maybe by regulating cytokine,it is suitable for clinical application.
