经纤维支气管镜肺灌洗治疗慢性阻塞性肺疾病急性加重期的临床评价

目的：评价经纤维支气管镜（纤支镜）肺灌洗治疗慢性阻塞性肺疾病急性加重期（AEc0PD）的临床疗效及安全性。方法：选择符合标准的85例AEcOPD住院患者，随机分为常规治疗组（A组，n=42）和灌洗组（B组，n=43），B组在常规治疗组基础上于住院后前3天进行灌洗治疗，两组分别于入院时和治疗后第5天行动脉血气分析，肺通气功能检查，并进行临床综合评价。结果：B组及A组治疗后的FEV1、PaO2、PaCO2、临床症状均明显改善（P〈0．05），B组较A组改善显著（P〈0．05）。结论：纤维支气管镜肺灌洗治疗AECOPD疗效确切，安全可靠，易于推广。

Effects of lung lavage by bronchofcopy in treatment of acute exacerbation of chronic obstructive pulmonary disease

Objective: To investigate the effects of lung lavage by bronchoscopy in the treatment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and its salty. Methods t85 patients with AECOPD were recruited on general wards and randomly assigned to standard medical treatment group(group A, n = 42)or early administration of lung lavage Plus standard medical treatment group (group B, n = 43). Lung lavage was performed in the group B with Lung lavage fluid on the three days prior to admission. Arterial blood gases, respiratory function and comprehensive assesment of clinical data were observed before and on the 5th day after treatment. Results:The levels of FEV1, PaP2, PaCO2 and clinical symptoms were markedly improved after A. B group treatment. Group A and B had statistically significant difference in the two group(P< 0.05). Concluions: Lung lavage by hronchoscopy was a safe and effectual way and easy to popularize in the treatment of AECOPD.
