彩色多普勒超声对孔源性视网膜脱离术后眼底血流动力学的研究

摘    要:目的 观察孔源性视网膜脱离患眼施行不同的手术方式对眼部血流动力学的影响。方法 采用彩色超声多普勒技术分别检测 40例孔源性视网膜脱离患者术前、术后不同时期患眼及对侧健眼视网膜中央动脉 (CRA)、睫状后短动脉 (PCA)、眼动脉 (OA)血流及阻力指数 (RI) ,对侧健眼为对照组。结果 术前患眼与对照眼的CRA、OA、PCA血流的各项指标比较 ,差异无统计学意义。术后 7天患眼CRA及PCA的Vmax、Vmin下降明显 ,RI升高 ,P<0 .0 5 ,差异有统计学意义 ;其中环扎加压组与加压组比较 ,PCA、CRA的Vmax、Vmin下降更明显 (P <0 .0 5 )。术后患眼与对照眼OA血流的各项指标比较 ,差异无显著性 (P >0 .0 5 )。结论 局部外加压术和环扎术均可以引起CRA、PCA血流速度的改变 ,以环扎术后CRA血流速度改变较为明显

Abstract:Objective To observe the influence of different operations for patients with eyes with rhegmatogenous retinal detachment on the hemodynamics of the eye. Methods To examine the sick eyes of patients with rhegmatogenous retinal detachment before and after operations, and blood flow and the resistant index (RI) of the central retinal artery (CRA), short posterior ciliary artery (PCA) and the ophthalmic artery (OA) of the healthy eyes, the healthy eyes being in the control group. Results A comparison of the indexes of CRA, OA and PCA in both the sick eyes before the operations and the eyes in the control group found the differences among them no statistically significant. The Vmax and Vmin of the CRA and PCA of the sick eyes dropped greatly while the RI increased seven days after the operations, P<0.05, the difference being significant statistically. Among them a comparison between the encircling and buckling group and the buckling group found the Vmax and Vmin of the CRA and PCA dropped even more greatly (P<0.05). A comparison of the indexes of the OA blood flow between the sick eyes and healthy eyes after the operations found no noticeable difference(P>0.05). Conclusions Both scleral buckling and encircling can cause the change of blood flow velocity of CRA and PCA, but the change after encircling being more notable.
