血管内介入治疗消化道出血的临床应用

目的评价PSA血管造影诊断和介入治疗消化道出血的临床应用价值。方法对62例消化道出血患者分别行腹腔动脉、肠系膜上动脉、肠系膜下动脉DSA造影检查。DSA表现有明确出血的采用栓塞治疗，不宜栓塞部位出血的行垂体后叶素灌注治疗。随访1～3个月。结果62例中43例血管有异常表现，表现为造影剂外溢征23例、新生肿瘤血管和肿瘤染色征3例、动脉瘤4例、血管畸形13例，43例中，栓塞治疗27例，有明确血管出血的23例、血管异常4例。药物灌注治疗29例，血管异常药物灌注治疗16例。无血管异常灌注血管加压素13例。结论DSA对消化道出血的诊断具有较高价值；动脉栓塞或药物灌注治疗出血性病变创伤小、安全、高效。

The Clinical Application of Digital Subtraction Angiography in Gastrointestinal Hemorrhage

Objective： To evaluate the clinical value of Digital Subtraction Angiography and interventional treatment of Gastrointestinal Hemorrhage Methods： DSA of celiac, superior mesenteric and inferior mesenteric arterise were performed for 62 patients of Gastrointestinal Hemorrhage, according to the angiograms,embolization were used for patients with obvious hemorrhage, Vasopressin infusion were used for patients inadvisable to embolism. Follow-up time was one to three months.Results 43 out of the 62 patients＇ .blood vessels have aberrant manifest, including 23 patients with contrast medium overflow, 3 patients with newborn tumor vessel and tumor stain, 4 patients with aneurysm and 13 patients with Vascular Abnormalities. Out of the 43 patients, there are 27 patients cured by embolism, 23 patients with obvious hemorrhage of blood vessels and 4 patients with aberrant blood vessels. There are 29 patients cured by medicine perfusion and 16 patients by medicine perfusion of aberrant blood vessels. 13 patients are cured by vasopressin perfusion without aberrant blood vessels. Conclusion DSA is of great value in diagnosis of the gastrointestinal hemorrhage. Arterial embolization or medicine infusion are effective and safe methods with small trauma to hemorrhage.
