急性缺血性卒中患者病因分析

目的依据改良TOAST分型方法作为标准,探讨河北省中南部地区急性缺血性卒中的病因学特征。方法收集2002年1月至2007年6月入住我院神经内科的急性缺血性脑卒中患者作为研究对象,依据改良的TOAST病因分型法作为诊断标准,结合头颅DWI和/或TCD微栓子监测结果确定动脉粥样硬化性血栓(AT)的诊断及病因分型。结果本研究共纳入1164例急性缺血性卒中患者,所有患者M-TOAST亚型构成比例分别为:动脉粥样硬化性血栓71.48%,小动脉病变21.39%,心源性栓塞3.09%,其它病因明确0.86%,不明病因者3.18%。结论河北省中南部地区急性缺血性卒中的病因分型以AT为主。

The etiological analysis of acute ischemic stroke

Objective To determine the etiological characteristics of acute ischemic stroke in Chinese people on the basis of modified Trial of Org 10172 in Acute Stroke Treatment(Modified TOAST) criteria.Methods From January 2002 to June 2007,1164 patients with acute ischemic stroke were enrolled in the study,who were the inpatients in department of neurology,the third hospital of Hebei medical university.The ischemic stroke was classified into five subtypes following modified TOAST criteria.The judgment of atherothrombosis was made after referring cranial diffusion weighted imaging(DWI) and microembolic signal(MES) monitored by transcranial doppler.Results The percentage of each modified TOAST subtypes was as following: atherothrombosis 71.48%,small-artery occlusion lacunars 21.39%,cardioembolism 3.09%,stroke of other demonstrated etiology 0.86%,stroke of other unknown etiology 3.18%.Conclusions Atherothrombosis is the most common etiology classification of ischemic stroke in Chinese people.The percentage is higher than that of other reports before.
