腹腔镜直肠癌根治术96例疗效分析

目的 评价总结腹腔镜下结直肠癌根治术的优点、特点、技巧及方法。方法 对96例直肠癌患者采用腹腔镜辅助直肠癌根治手术，并对其临床资料、手术方式及随访结果进行总结。结果 本组96例，84例行直肠癌全直肠系膜切除（TME），结直肠／或结肛吻合术；12例行腹会阴联合切除术（Miles）；术中平均出血量86．5ml，平均手术时间150min。平均清除淋巴结数为12．3枚。吻合口漏3例。腹腔内出血1例。会阴疝1例。本组患者全部康复出院，最长随访46个月，局部复发3例。结论 腹腔镜下结直肠癌根治手术，安全可靠，出血量少，肠功能恢复快。

Laparoscopic curative rectectomy for rectal cancer

Objective To summarize the advantages, characteristics, skills and methods of the laparoscopic curative rectectomy for rectal cancer. Methods Laparoscopic curative rectectomy for rectal cancer was performed in patients with rectal cancer from Aug 2000 to Aug 2005; The clinical data operation methods and the follow-up data were analyzed. Results Among the 96 patients, 84 received TME, end to end anastomosis of the colon and rectum or end to end anastomosis of colon and anus; 12 received Miles operation. The amount of bleeding during operation was 86.5 ml and the average operation time was 150 minutes. The average lymphadenopath were 12.3 nodes. Anastomosis leakage occurred in 3 patients. One patient had abdominal bleeding and 1 had perineal hernia. All 96 patients recovered. The longest follow up was46 months, There patients had local recurrence. Conclusion Laparoscopic curative rectectomy for rectal cancer is safe with less bleeding and quick recovery of intestinal function.
