尿NAG、mALB、β_2-MG、α_1-MG检测对早期肾损害的诊断价值

摘    要:目的探讨尿N-乙酰-β-D-氨基葡萄糖苷酶(NAG)、微量白蛋白(mALB)、β2-微球蛋白(β2-MG)、α1-微量球蛋白(α1-MG)检测对早期肾损害的诊断价值及临床意义。方法将150例受试者分为健康对照组(为门诊健康体检人群)和病例组(均为我科门诊和住院患者)。分别检测两组尿NAG、mALB、β2-MG、α1-MG的含量,并对检测结果进行比较及统计学分析。结果病例组检测的阳性率及4项检测指标结果均显著高于健康对照组,病例组4项检测指标尿NAG、β2-MG、mALB、α1-MG的阳性率分别为82.1%、70.5%、64.2%和41.0%,尿NAG的敏感性高于尿mALB、β2-MG、α1-MG,差异有统计学意义(P〈0.001)。尿β2-MG的敏感性高于尿mALB、α1-MG,差异有统计学意义(P〈0.05)。结论尿NAG、β2-MG检测对早期肾损害更具有敏感性,可作为肾损害性疾病的早期诊断、疗效观察的有效指标。

Abstract:Objective To investigate the diagnostic value and clinical significance of urinary NAG,mALB,β2-MG and α1-MG in early renal injury.Methods 150 subjects were divided into control group(healthy people for the outpatient) and case group(all my outpatients and inpatients).The urinary NAG,mALB,β2-MG and α1-MG were measured.Results The positive rate and NAG,mALB,β2-MG and α1-MG were significantly higher than that of the control group.The positive rates of urinary NAG,MALB,β2-MG and α1-MG were 82.1%,70.5%,64.2% and 41.0%,respectively.The sensitivity of urinary NAG was higher than that of the other three group(P0.001).The urinary β2-MG was more sensitive than urinary mALB and α1-MG(P0.05).Conclusions Urinary NAG and β2-MG can be used as the sensitive indicates of the diagnosis and therapeutic effective in early renal injury.
