下肢深静脉血栓形成时D-二聚体监测的临床意义

摘    要:
目的 探讨D-二聚体变化在下肢深静脉血栓形成的临床意义。方法 回顾性分析2004年6月至2006年6月89例下肢深静脉血栓形成患者临床资料。结果 全部患者治疗后下肢肿胀缓解,治疗前患者D-二聚体含量均高于正常值,治疗后第一天D-二聚体明显增高,与入院检查比较差异有显著性(P〈0.05)。治疗后1、2周呈逐渐下降,与入院检查比较差异有显著性(P〈0.05),治疗结束后D-二聚体含量均在正常范围之内。下肢血管彩超证实混合型中2例全部再通,15例部分再通,中央型中5例再通,11例部分再通,周围型7例再通,部分再通11例。结论 D-二聚体可作为诊断急性DVT的特异性指标,监测D-二聚体可以指导治疗用药,根据D-二聚体的变化制定个性化治疗方案。

Abstract:
Objective To discuss clinical significance of D-dimmer changes in deep venous thrombosis(DVT) of lower extremity.Methods The clinical data of 89 cases of lower extremity DVT during June 2004 to June 2006 were reviewed.Results The swelling of lower extremity in all of the patients disappeared.The values of D-dimmer pretreatment were higher than normal.They were significantly increased in the first treatment day,and had statistical significance than pretreatment(P<0.05),but were gradually decreased in 1st or 2nd weeks treatment.They were in normal after the treatment.The vessel colorful ultrasonogram showed that 2 cases had recanalization and 15 cases had partial recanalization in mixed DVT,5 cases recanalized and 11 cases partially recanalized in cental DVT,while 7 cases racanalized and 11 cases partially recanalized in peripheral DVT.Conclusion D-dimmer can be viewed as the special indicator of diagnosing acute DVT.To screen the d-dimmer can instruct the drug therapy and establish the individual therapy strategy.
