脊柱结核椎间植骨、单节段内固定的生物力学测试

摘    要:目的测试椎间植骨后脊柱结核单节段短椎弓根螺钉内固定手术模型的生物力学稳定性。方法使用10具小牛脊柱标本(T12-L3)模拟体外L1-2椎间盘周围型脊柱结核前路病变椎体切除手术,即切除L1-2间盘和邻近终板,行自体髂骨植骨、后路用单节段短椎弓根螺钉进行固定。测试模型对照组、植骨组和非植骨组在垂直压缩、侧弯、屈伸和旋转方向上的力学稳定性。结果在垂直压缩、侧弯、前屈和后伸载荷下植骨组的相对运动范围(RROM)明显小于非植骨组(P〈0.05),在扭转载荷下两组相比无明显差异(P〉0.05);在轴向载荷方向上非植骨组后路内固定棒的应变较植骨组明显增加。结论椎间植骨后可明显增加模型在垂直压缩、侧弯、前屈后伸方向上的稳定性,减少后路内固定棒的载荷分怖,然而在非融合状态下,对于扭转的影响不大。
