摘    要:目的探讨慢性乙肝患者免疫状态对HBsAg和HBVDNA水平的影响。方法以慢性乙肝患者作为研究对象采集其外周血,采用ELISA法和流式细胞仪分别对研究对象血清及全血进行乙肝病毒血清学标志物HBsAg、IL-2、IL-10和淋巴细胞亚群CD4^+和CD8^+检测,应用PCR法对HBVDNA进行检测,并对HBVDNA指数结果进行对数转换。应用SPSS13.0 for window统计软件进行相关性分析。结果IL-2与HBsAg呈无显著性负相关关系(r=-0.064,P=0.547),但与HBV DNA水平呈显著性负相关关系(r=-0.261,P=0.013);IL-10与HBsAg水平呈无显著性负相关关系(r=-0.195,P=0.064),与HBVDNA水平呈无显著性正相关关系(r=0.015,P=0.888);IL-2/IL-10比值与HBsAg水平呈无显著性相关关系(r=0.087,P=0.410),与HBVDNA水平呈无显著性负相关关系(r=-0.195,P=0.063);CD4^+细胞与HBsAg及HBVDNA水平均呈无显著性正相关关系(r=0.188,P=0.074;r=0.051,P=0.632);CD8^+细胞也与HBsAg水平呈无显著性负相关关系(r=-0.107,P=0.315),与HBVDNA水平呈无显著性正相关关系(r=0.146,P=0.169);CD4^+/CD8^+比值与HBsAg水平呈无显著性正相关关系(r=0.158,P=0.134);但与HBVDNA水平呈低负相关关系(r=-0.201,P=0.055)。结论慢乙肝患者体内IL-2对HBsAg水平虽无显著抑制作用,但对HBVDNA的复制起到显著抑制作用;IL-10对HBsAg及HBVDN水平均无显著影响,IL-2/IL-10之间的平衡对HBVDNA复制亦无明显影响;CD4^+、CD8^+细胞对HBsAg及HBVDNA水平虽无明显影响,但CD4^+/CD8^+细胞之间的平衡似对HBV的复制有抑制作用。

Abstract:Objective To investigate the influence of immune state on the level of HBsAg and HBVDNA in the patients with chronic hepatitis B.Methods The peripheral blood was collected from the patients with chronic hepatitis B,IL-2,IL-10 and the marks of hepatits B virus infecting in the patient sera were tested by ELISA,CD4^+ and CD8^+ T-lymphocytes were examined by flow cytometer (EPICS XL).HBVDNA was examined by PCR and the index numbers of HBVDNA were changed into constants by logarithm. All the data was calculated by software of SPSS 13.0 for window.Results There was no significant negative relationship between IL-2 and HBsAg (r=-0.064,P=0.547),but significant negative relation to the HBVDNA(r=-0.261,P=0.013).There was no significant negative relationship between IL-10 and HbsAg(r=-0.195,P=0.064),also no significant relation to HBVDNA(r=0.015,P=0.888).there also no significant relationship between the rate of IL-2/IL-10 and the level of HBsAg (r=0.087,P=0.410),no significant negative relation to the level of HBVDNA(r=-0.195,P=0.063).There were also showed that no significant relationship between CD4^+ T-lymphocyte and HBsAg and HBVDNA (r=0.188,P=0.074;r=0.051,P=0.632);No significant negative relationship between CD8^+ T-lymphocyte and the HbsAg(r=-0.107,P=0.315),and no significant relation to HBVDNA (r=0.146,P=0.169);No significant relationship between the rate of CD4^+/CD8^+ T-lymphocyte and the level of HBsAg was manifested,but significant negative relation to t HBVDNA.Conclusions In patients with chronic hepatitis B,IL-2 could not affect on the level of HBsAg,but might influence on the replication of HBV.IL-10 exerts no effect on the level of HBsAg and HBVDNA;the balance of IL-2/IL-10 plays no role on the level of HBsAg and HBVDNA.CD4^+ and CD8^+ T-lymphocytes play also no part in the level of HBsAg and HBVDNA,but the balance of CD4^+/CD8^+ T-lymphocyte might influence on the replication of HBV.
